On 11/29/2021 at 1:26 PM, Licensing Program Analyst (LPA) Austin Rios conducted a Case Management inspection. LPA met with Director Laurie Pido and advised director the purpose of the inspection. A tour of the facility was conducted by LPA with Director. The facility had 51 children in care and 17 staff at the time of the inspection.

On 11/23/2021 the facility sent out a letter to parents of the school regarding an incident of a registered sex offender being arrested who volunteered in the premises of the church.

Interview with Director revealed the incident was never self-reported to the department. LPA reminded Director that center staff are Mandated Reporters and are required by law to report suspected cases of abuse or neglect to CPS and CCL. Director stated she understood and will be submitting LIC624 to the CCL to include information regarding the incident.

The following CCR, Title 22, Division 12 regulation was cited 101212(a)(d)(1)(D) Reporting Requirements

LPA Rios explained and provided appeal rights to the Director. One type B deficiency was cited today: Reporting Requirements

Director has submitted an incident report and outline a plan to eliminate the occurrence of no reported incidents to CCLD.

This requirement was not met as evidenced by on or around the week of 11/22/2021, the facility was made aware that a church volunteer and/or tech volunteer was arrested for crimes against a minor child whom attends the Ventura Missionary Private School. The facility failed to report the unsusual incident and the arrest of the church volunteer. The church is in a different dwelling, however is located on the same property of the licensed preschool center.
This poses a potential health, safety or personal rights risks to persons in care.
