This Case management visit was conducted to address the deficiencies noted during complaint control # 29-AS-20210524120011 investigation visit conducted on 5/25/21.

During facility tour on 5/25/21 starting at 10:59 AM with the Administrator LPA observed metal polish, disinfectant, stainless steel cleaner, furniture polish and air freshener in an unlocked 1st floor laundry room, alcohol prep pads, sani cloth bleach germicidal disposable wipes, florajen over 20 B cells capsules belonging to resident #1 (R1) in a refrigerator, disinfecting wipes, glass cleaner, comet cleanser, air freshener and sani-cloth germicidal disposable wipes in a memory care unit office, shovel, rake, gasoline, CPVC, PVC purple primer, pipe cement, Monterey Neem Oil funicide miticide and insecticide, power steering fluid, rose and flower care fertilizer insect control disease control concentrate in backyard area of memory care unit, tree pruner in backyard area of assisted living and sani-cloth germicidal disposable wipes on top of a medication cart on the 2nd floor all accessible to the residents. LPA reviewed resident records starting at 1:58 pm and obtained copies of pertinent documents.


Pursuant to Title 22 Division 6 Chapter 8 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 809-D):
