Licensing Program Analyst (LPA) Valeria Conway arrived at the facility unannounced to conduct a required annual visit at 9:30 A.M. The LPA met with staff member, Maria Elena Villarico. LPA explained the reason for today's visit. The Administrator, Louisa Nolan was contacted via telephone and arrived at 10:45 A.M. The Administrator authorized the LPA to conduct the physical plant tour with the staff member present. Entrance interview conducted.

The facility is vendored through Tri-Counties Regional Center as a level 3 home. No clients were present at the time of LPA arrival. Only one (1) staff was present. The Facility does handle cash resources for some of the clients. Surety bond is current.

Beginning at 9:55 A.M., the LPA along with the staff member toured the physical plant areas inside and outside to ensure there are no health and safety hazards and the facility is in compliance with Title 22 Regulations. The following was observed:

Individual smoke alarms and separate carbon monoxide detectors were tested between 10:00 A.M. - 10:25 A.M. and all functioned properly. The fire extinguisher was observed to be fully charged and last purchased on 03/11/2025.

OUTDOOR SPACE: The backyard patio contains a shaded area and is equipped with furniture for clients' use. An outdoor shed was observed to be locked. All passageways and exits were observed to be clear and free of hazards. Facility has two total gates; both were observed to be self-closing and self-latching.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

COMMON SPACES: Living room, dining room, and activity room furniture was observed to be in good condition. The LPA observed the required postings upon entry and throughout the common areas. The facility maintained a comfortable temperature. Facility provides sufficient space to accommodate both indoor and outdoor activities. LPA observed a working phone available for clients use whenever needed. Cash resources are kept locked in a file cabinet located in the activity room.

BEDROOMS: The LPA observed three shared client bedrooms, which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. LPA observed that the closet doors in Room #3 were missing. The Administrator stated that the doors were removed because they were coming off the track and posed a potential safety hazard to clients. The Administrator reported that the doors will be replaced with curtains and a photo documentation will be submitted to the LPA. The facility also contains a locked staff room.

BATHROOMS: The LPA observed three (3) shared bathrooms. Client bathrooms were observed to be clean and sanitary and in operating condition. Between 10:03A.M. and 10:10 A.M hot water was checked in the shared client restrooms and measured between 120.4-122.0 degrees Fahrenheit.

KITCHEN: Knives are stored in a locked drawer. Kitchen appliances appeared to be in operable condition. The facility has a sufficient supply of perishable and non-perishable food. LPA conducted a review of expiration dates on product labels. At 9:58 A.M. hot water measured 125.1 degrees Fahrenheit. Administrator stated that someone raised the main water heater temperature without permission.

GARAGE: LPA observed the facility garage which contained extra food, locked cleaning supplies, locked laundry supplies, emergency food and water as well as the laundry area.

STAFF FILES: At 11:35 A.M. six (6) staff files including the Administrator’s were reviewed. Files were reviewed for, but not limited to: fingerprint background clearance, health screening, TB test, and training records. Files for five (5) staff were complete. Staff #1’s personnel file was removed from the facility and taken offsite in error; therefore LPA was not able to review it at the time of the visit. Technical Violation (TV) issued. LPA observed that three (3) staff members were behind on training.

CLIENT FILES/CASH RESOURCES: At 12:03 P.M. LPA reviewed six (6) client files for, but not limited to: physician's report, proof of TB test, Admission Agreement, annual service plans/IPPs, medical assessments, and cash resources. All six (6) client files and cash ledgers were observed in compliance with regulation.

MEDICATION REVIEW: Beginning at 12:56 P.M, LPA attempted to review medications for six (6) clients. LPA reviewed medications for four (4) clients only, as the Centrally Stored Medication and Destruction Log forms for Client #1 (C1) and Client #2 (C2) were not available on site. The medication reviews that were completed were documented and administered in compliance with regulation. LPA explained to the Administrator that if required medication logs are missing and the LPA is unable to verify medication administration or disposal in future visits, a medication related citation will be issued.

INFECTION CONTROL/EMERGENCY DISASTER PLANNING: During today’s visit, the LPA reviewed the facility's infection control practices and the facility's emergency disaster plan. The facility’s policies and procedures as they pertain to infection control are adequate. Emergency disaster drills are conducted monthly, with the last drill documented on 12/11/2025. Emergency disaster plan was observed to be complete and updated annually as required.

During today’s visit, the LPA gathered the following documents: Personnel Record (LIC 500), Register of facility Clients/Residents LIC 9020, Liability Insurance and Surety Bond.

INTERVIEWS: Throughout the duration of today's visit, all clients were out of the facility. LPA interview one (1) staff member.

Pursuant to Title 22, California Code of Regulations and/or CA Health and Safety Code, the following deficiencies were cited (refer to LIC 809-D.) Administrator was informed that failure to correct the deficiencies may result in civil penalties.

An immediate civil penalty of $1,000 repeat violation is assessed today due to being cited for the same violation within 12 months. Administrator was informed that additional civil penalties might be assessed based on Health and Safety Code 1569.49(e) and 1569.49(f).

Exit interview conducted. A copy of the report and appeal rights were provided.
