CLIENT FILES/CASH RESOURCES: At 12:03 P.M. LPA reviewed six (6) client files for, but not limited to: physician's report, proof of TB test, Admission Agreement, annual service plans/IPPs, medical assessments, and cash resources. All six (6) client files and cash ledgers were observed in compliance with regulation.
MEDICATION REVIEW: Beginning at 12:56 P.M, LPA attempted to review medications for six (6) clients. LPA reviewed medications for four (4) clients only, as the Centrally Stored Medication and Destruction Log forms for Client #1 (C1) and Client #2 (C2) were not available on site. The medication reviews that were completed were documented and administered in compliance with regulation. LPA explained to the Administrator that if required medication logs are missing and the LPA is unable to verify medication administration or disposal in future visits, a medication related citation will be issued.
INFECTION CONTROL/EMERGENCY DISASTER PLANNING: During today’s visit, the LPA reviewed the facility's infection control practices and the facility's emergency disaster plan. The facility’s policies and procedures as they pertain to infection control are adequate. Emergency disaster drills are conducted monthly, with the last drill documented on 12/11/2025. Emergency disaster plan was observed to be complete and updated annually as required.
During today’s visit, the LPA gathered the following documents: Personnel Record (LIC 500), Register of facility Clients/Residents LIC 9020, Liability Insurance and Surety Bond.
INTERVIEWS: Throughout the duration of today's visit, all clients were out of the facility. LPA interview one (1) staff member.
Pursuant to Title 22, California Code of Regulations and/or CA Health and Safety Code, the following deficiencies were cited (refer to LIC 809-D.) Administrator was informed that failure to correct the deficiencies may result in civil penalties.
An immediate civil penalty of $1,000 repeat violation is assessed today due to being cited for the same violation within 12 months. Administrator was informed that additional civil penalties might be assessed based on Health and Safety Code 1569.49(e) and 1569.49(f).
Exit interview conducted. A copy of the report and appeal rights were provided.
