BATHROOMS: All restrooms were clean and sanitary and in operating condition with grab bars and slip-resistant surfaces. The bathrooms were sufficiently stocked with supplies and paper towels. The hot water temperature was measured in each restroom and measured within regulation standard of 105 - 120 degrees Fahrenheit.
COMMON AREAS: LPA observed the living room and dining room area to be furnished appropriately, and all furniture was observed to be in good condition at the time of the visit. The facility maintained a comfortable temperature. LPA observed required postings throughout the common space. Activities were observed in the living room. There is a working telephone on premises. At 9:10 A.M. combination smoke and carbon monoxide detectors were tested, and all were functional at the time of the visit. LPA observed fire extinguisher to be fully charged and purchased in 04/26/2026. The property has an enclosed patio area where a staff refrigerator, extra food storage, emergency food and water supply were observed. Adjacent to this room, LPA observed room with an office area.
OUTDOOR/BACKYARD: There is a shaded area in the backyard with appropriate furniture for client use. LPA observed multiple self-latching gates for emergency use. The exterior passageways were clean and clear of any obstructions at the time of the visit. No bodies of water were observed. The perimeter is fully gated. Extra emergency water tanks were observed outside the enclosed patio area. Two (2) sheds were observed locked containing gardening tools and storage space.
LAUNDRY ROOM: Facility has a designated area where the washer and dryer are kept. Detergents and cleaning solutions were observed in locked cabinets.
GARAGE: According to the administrator, the garage was converted into an ADU and it is inaccessible to clients in care.
RECORD REVIEW: Records review, four (4) client records were reviewed for, but not limited to: appraisals, medical records, admissions agreement, consent forms. Three (3) Personnel records were reviewed for, but not limited to: personnel records, health assessments, criminal record clearances, first aid/CPR training, and the appropriate training. All files were observed to be in order at this time. Last emergency disaster drill was conducted on 06/27/2026
