During today’s inspection, LPAs interviewed staff #3, staff # 4 and the E.C.E Coordinator. According to the E.C.E Coordinator child #1 did not need medical attention and child #1 is still enrolled in the program. Also staff interviews revealed multiple incidents have occurred in the past involving child #1's behavior. The center staff failed to report these incidents to Community Care Licensing.

LPAs reviewed child #1's and staff files and interviewed staff which revealed no incidents were documented in the child #1's file.

LPAs advised, the center will be receiving type B deficiencies for failure to report incidents involving child #1 to Community Care Licensing within 24 hours as required. In addition, for violation of child's personal rights.

Exit interview was conducted, report reviewed, and copy was provided to E.C.E coordinator. Notice of site visit was given.

THE NOTICE OF SITE VISIT WAS POSTED AS REQUIRED BY H&S CODE SEC. 1596.817. THE NOTICE OF SITE VISIT MUST REMAIN POSTED FOR 30 DAYS OR A CIVIL PENALTY OF $100.00 WILL APPLY.
