32 | and the same parent stated how the facility should have more skilled staff, in order to provide constant care and supervision.
Additionally, LPA reviewed staff qualifications, which included college transcripts, LIC 501, immunization records, LIC 503, LIC 508, LIC 9108, CPR certification, and the Mandated Reporter certificate (See LIC812). LPA confirmed that all the staff reviewed had a current Mandated Reporter certificate. Furthermore, all staff designated as teachers currently have sufficient Early Childhood Education (ECE) or Child Development units.
LPA also interviewed the director, staff, and personnel associated with the facility. The director stated during the interviews that she had not observed unqualified staff providing care and supervision. However, after interviewing multiple staff members and personnel, LPA discovered that there have been multiple witnesses who observed a unqualified staff providing care and supervision. Specifically, the LPA received a signed declaration (LIC 855) from a witness who is/was associated with the facility. The declaration states that on 11/4/2024, at approximately 2:00 PM, S3 (an unqualified staff member) was left alone providing care and supervision to the preschool program. Additionally, the LPA conducted another interview with a staff member who corroborated the aforementioned incident on 11/4/2024.
Based on LPA interviews, documents collected during investigation, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 & Chapter 1), are being cited on the attached LIC 9099D.”
