25 | On November 3, 2022 11:00 AM Licensing Program Analyst (LPA), Laura Villanueva conducted an unannounced inspection to complete a Case Management-Other visit. Prior to inspection, LPA Villanueva asked pre screening questions related to COVID 19, Director's response indicates there are no COVID 19 exposures on site. LPA toured classrooms with Director. There were 53 children with 6 staff.
On October 11, 2022 an incident occurred that was reported to the Department involving 2 children.. The 2 year old class was outside at 10:45 AM with 2 teachers and 17 children. C1 was riding a car when C2 came up to her and tried to take the car. C1 told C2 no. C2 pushed C1 off the car. C1 fell back and hit her head on a wall resulting in an open wound on the back of her head. T1 informed T2 of the incident and Director was called to assistant. C1's wound was bleeding, so the teachers cleaned the area. C1's mother was called. C1 was taken to the hospital where she had staples placed on the cut. C1 was out from school for 1 day. C2's mother was called to pick him up due to aggressive behavior. C2 retuned to school the following day. Teachers continue to monitor C2 and communicate with him and his mother about pushing other children.
A plan for C2 is now in place. The Director and Teachers are shadowing C2 more closely and are providing written incident reports to parents when he is aggressive toward other children.
Exit interview conducted with Director, Katherine Eclarinal and a copy of this report was given.No deficiencies cited today.
