32 | All parents interviewed stated, they were happy with the care and supervision. However there was one parent who stated they have witnessed during drop off time, 7 infants and only one staff member.
Additionally, LPA interviewed the director, staff, and personnel/witnesses who were/are currently associated with the facility. From interviewing witness#7 and witness#16, LPA gathered that this facility does not always maintain proper ratios as outlined in Title 22 California Code of Regulations.
As mentioned, LPA completed a review of the infant sign-out sheets and staff time cards/schedules. LPA discovered the following discrepancies: On 12/06/2024, from 1:11PM to 1:48PM S16 and S17 were on break, it is unclear who supervised the 15 infants in attendance(See LIC812) . Also, on 10/29/2024, from 11AM to 11:45AM there were 16 infants in attendance. It is unclear who supervised the 16 infants(see LIC812). Additionally On 10/24/2024 there were 17 total infants in attendance, it is unclear who supervised the 17 infants (see LIC812). Lastly on 9/23/2024 from 12PM to 1PM there are 17 infants in attendance, it is unclear who supervised the 17 infants. Today LPA asked the director to provide documentation or evidence that would clearly show the facility was in compliance with ratios on 12/06/2024 and 10/29/2024, 10/24/2024 and 9/23/2024 the director could not provide any documentation or evidence (See LIC812).
Based on LPA interviews, documents collected during investigation, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 & Chapter 1), are being cited on the attached LIC 9099D.”
