25 | On October 19, 2022 at 11:47 AM , Licensing Program Analyst (LPA) ) Rona Chavez conducted an unannounced Case Management inspection. LPA met with facility Director Rebecca Hernandez and advised her the purpose of the inspection. Director provided LPA a tour of the facility inside and out. There were 30 children in care and 5 staff at the time of the inspection.
On 10/10/2022 facility self reported that on 10/06/2022 at transition to outside play at around 11 Am Child 1 (C1) and Child 2 (C2) were lining up to transition outside and were role playing they were getting married. Child 3 (C3) told Staff 1 (S1) that C1 and C2 kissed. S1 reported to director that she did not see them kiss but did hear that they were playing wedding. On 10/7/2022 C2's called and reported that her child told her she kissed while she was playing wedding at school. Director was unaware of the incident as S1 had not reported it to her. Director advised S1 that all incidents must be reported to Director and/or parent in a timely manner. Will review reporting requirement with teacher.
Today, LPA inspected the class room and observed where the incident took place as well as interview staff. The Director reported the incident and submitted the incident report to the Department as required.
No deficiencies were cited during today's inspection.
Failure to Post the Notice of Site Visit for 30 days may result in a $100.00 Civil Penalty.
