On 3/03/2026 at 11:16AM PST, Licensing Program Analyst (LPA) Seena Parsapour conducted an
unannounced Complaint Investigation inspection of the abovementioned Child Care Center (CCC) for
the purpose of delivering findings pertaining to the above listed allegations. LPA met with Director, Vivian Bennett, and explained the nature & purpose of the inspection.
LPA, in the company of Director, toured the interior & exterior of the CCC in its entirety.
The investigation included two unannounced inspections, record reviews, and interviews. Interviews were conducted with CCC staff and parents of children currently and formerly enrolled, by LPAs Seena Parsapour & Shane Loftus. As noted above, the specific allegation of the complaint is that staff do not meet children’s diapering needs.
LPA was able to corroborate the allegation of the complaint through parent interviews, which revealed a pattern of improper diapering practices. Namely, interviews with parents revealed repeated instances in which staff failed to change soiled diapers, resulting in children developing diaper rash. (Cont. 9099-C, Page 2)

Based on the totality of the evidence gathered throughout the course of the investigation, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.

During today's inspection, one (1) Type A Deficiency is being cited under Title 22 of the California Code
of Regulations, and Health and Safety Codes (See LIC9099D).

Upon receipt, Director was informed that they must provide copies of this licensing report to each
parent/guardian of enrolled children, and to parents/guardians of newly enrolled children during the next 12
months. Acknowledgement of Receipt (LIC 9224) form shall be used for this purpose. LIC 9224, once
completed, shall be maintained in each child's file. (LIC 9224 was provided to Director).

Appeal Rights were provided & explained to Director. A notice of site visit was given and must remain posted
for 30 days. Exit interview conducted and report was reviewed with the Director, Vivian Bennett.

Based on interviews, the Licensee did not comply with the section cited above in that they did not ensure that each infant's diapers are being changed as often as necessary to ensure that the infant is clean and dry at all times, which poses an immediate health, safety, or personal rights risk to persons in care.

The investigation included two unannounced inspections, record reviews, and interviews. Interviews were conducted with CCC staff and parents of children currently and formerly enrolled, by LPAs Seena Parsapour & Shane Loftus.
Interviews, inspections, and documents reviewed did not corroborate the allegations noted above. LPA’s observations and interviews found that children in care are being properly cared for and supervised. Parents expressed overall satisfaction with staffing levels and reported teachers being respectful towards and interactive with children in care. LPA’s interviews furthermore reflected that parents are being properly notified when incidents involving their child occur at the CCC. (Cont. 9099-C, Page 2)

LPA found parents are immediately notified regarding incidents or injuries sustained while in care, or are otherwise notified upon pickup. Parents report that staff provide routine updates regarding their child(ren) in care and are happy with the communication they receive from staff.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur, therefore, the allegations are UNSUBSTANTIATED.
An exit interview was conducted with Director, Vivian Bennett. Director was provided with Appeal Rights (LIC 9058) and a Notice of Site Visit (LIC 9213). Notice of Site Visit must be posted for 30 days or a civil penalty of $100 may apply.
