13 | On July 10, 2024, at 10:15 AM Licensing Program Analyst (LPA) Laura Villanueva conducted an unannounced inspection to investigate the above allegation. LPA met with Director, Heather Shields and explained the purpose of the visit. LPA and Director conducted a tour of the facility inside and outside. LPA observed a total of 50 children under the care and supervision of 5 staff.
LPA interviewed Director, reviewed child files, and obtained a copy of the child roster. LPA obtained a copy of a letter from the pediatrition, a written statment from the parent, and immunization record. The child in the allegation is on a a catch up schedule for vaccinations. The doctor's letter details a schedule for administering vaccination starting in July 2024 and ending in December 2024.. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. Notice of Site Visit (LIC9213) will be posted. The notice shall be posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty. Appeal right given LIC9058. Exit interview conducted with Director, Heather Shields and a copy of this report was reviewed and given. |
