On 9/11/2026 at 9:30am, Licensing Program Analyst (LPA) Seena Parsapour conducted an unannounced Complaint Investigation Inspection at the abovementioned Child Care Center (CCC), for the purpose of delivering findings in reference to a complaint (control # 17-CC-20260810142159) received by the Department. LPA met with Director Vivian Bennett and explained the nature & purpose of the inspection. LPA toured the interior & exterior of the CCC, in the company of Director. At the time of the inspection, LPA observed seventeen (17) infant-aged children and six (6) staff members (cleared & associated).
The investigation included two (2) unannounced inspections & observations, interviews, and records review. Interviews were conducted with parents of children currently enrolled, Director, Assistant Director, and Staff #01 (S1). During the initial 10-day visit on 8/18/2026, LPA Parsapour obtained & reviewed records including the facility roster. During today’s inspection, records obtained & reviewed include a sampling of diapering logs, records pertaining to Staff #01 (S1), and records pertaining to S2. LPA interviewed S1 during today’s inspection, as well. (Cont. 9099-C, Page 2)

On 8/27/2026 at 9:30AM, Licensing Program Analyst (LPA) Seena Parsapour conducted an unannounced Complaint Investigation inspection of the abovementioned Child Care Center (CCC) for the purpose of delivering findings pertaining to the above listed allegation. LPA met with Director Vivian Bennett and explained the nature & purpose of the inspection. At the time of the inspection, LPA observed seventeen (17) infant-aged children under the care & supervision of six (6) staff members (cleared & associated).
The investigation included two (2) unannounced inspections & observations, interviews, and records review. Interviews were conducted with parents of children currently enrolled, Director, Assistant Director, and Staff #01 (S1). During the initial 10-day visit on 7/07/2026, LPA Parsapour obtained & reviewed records including the facility roster. During today’s inspection, records obtained & reviewed include a sampling of diapering logs, records pertaining to Staff #01 (S1), and records pertaining to S2. LPA interviewed S1 during today’s inspection, as well.
In regard to Allegation #2, inspections, observations, interviews, and records reviewed did not corroborate the allegation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore, the allegation is deemed UNSUBSTANTIATED.
No deficiencies were cited during today’s inspection.
A notice of site visit was given and must remain posted for 30 days. Appeal Rights were provided to Director. Exit interview conducted and report was reviewed with Director, Vivian Bennett.

During LPA’s interview with Director, Director indicated that on 8/10/2026, the parent (P1) of Child #01 (C1) brought it to Director’s attention that, while retrieving their child from the CCC on 8/07/2026, P1 observed C1 in the absence of direct visual supervision for several minutes. The incident is reported to have occurred on 8/07/2026 in the toddler play yard.

Based on interviews with CCC staff and records reviewed, it is determined that there was a lack of care and supervision provided at the time of the incident. LPA notes S2 is no longer employed at the CCC. Additionally, C1 is no longer attending the CCC.

LPA was able to corroborate the above-listed allegation through interviews and records review. Based on interviews and records review, the preponderance of evidence standard has been met, therefore the above-listed allegation is found to be SUBSTANTIATED.

One (1) type A deficiency was issued during today’s inspection (See LIC9099D for details). An immediate civil penalty of $500 was assessed.

LPA Seena Parsapour informed Director Vivian Bennett that this report dated 9/11/2026 documents one (1) Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Seena Parsapour informed the Director to provide a copy of this licensing report dated 9/11/2026 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A copy of this licensing report dated 9/11/2026 was provided to Director as well as a copy of the LIC9224 form. A notice of site visit was given and must remain posted for 30 days. Appeal rights were provided to Director. Exit interview conducted and report was reviewed with the Director, Vivian Bennett.
