25 | On 08/12/2024 a Case Management Inspection was conducted by LPA German Negrete who met with Director Viktoria Sherkanova. LPA advised the purpose of today's inspection was to review the incident which was reported to the Department on 05/22/2024 LPA conducted a tour of the facility inside and out with the director. LPA observed 69 children being supervised by 10 staff.
Director of the facility stated the following : On 5/22/2024 at 2:32 PM, Child#1 (5 years 5 months) attempted to jumped off the small play structure, child#1 lost coordination/balance and Child#1 landed on the right arm. The lead teacher, S#1 approached child#1 after observing the child crying. The director called the parent of the child#1, Parent#1 (an employee/teacher of the center) to come and Parent#1 saw Child#1 after the incident. Also director was able to show camera footage of the incident to Parent#1. In todays interview parent#1 stated and confirmed parent#1 was able to observe the camera footage of the incident.
Director submitted a LIC624 Unusual Incident Report, with in the appropriate time frame, per Tittle 22 Regulations. During todays inspection LPA observed the play yard(s). LPA documented photos of the play structures. LPA observed Director remove the play structure(play structure#1) from the facility, that Child#1 fell off . Also Director stated, play structure#1 was removed from the big yard immediately after the incident and was placed in the toddler yard. LPA received manufactures instructions for play structure#1.
Due to insufficient information available at the time of this inspection, the investigation will be on going.
Exit Interviewed conducted with Director . A notice of site visit was provided.
FAILURE TO POST THE NOTICE OF SITE VISIT FOR 30 DAYS MAY RESULT IN $100.00 CIVIL PENALTY
