32 | LPA interviewed staff members whom indicated the construction/remodeling had started the week prior and had been going on for more than 2 days. One staff member shared that the Licensee had previously done other remodeling in the home during operational hours.
Based on LPAs observations, interviews which were conducted, documents gathered and record review, the preponderance of evidence standard has been met, therefore the above allegation is found SUBSTANTIATED.
The following deficiencies are being cited in accordance to Title 22 of the California Code of Regulations and/or Health & Safety codes. Please refer to LIC9099D for documentation of deficiencies cited: 102416.3 Alterations to Existing Buildings or Grounds (a)(6)
A notice of site visit was given and must remain posted for 30 days.
Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
A copy of the Appeal Rights (LIC 9058 FAS 01/16) were given and explained. Licensee’s signature on this form acknowledges receipt of these rights.
