On June 1, 2023 at 10:35 AM Licensing Program Analyst (LPA) Laura Villanueva made an unannounced inspection to conclude the investigation for the allegations. LPA met with Director, Roshini Wirekoon explaining the purpose of the inspection. LPA observed a total of 20 preschoolers under the care and supervision of 3 staff.Based on LPA observations, video footage, interviews with staff and parents, and documentation. The Director, Roshini Wirekoon reported the incident of vermin present in the infant room and utility closet to the Department on 03/10/23. Director contacted Western Exterminator to set rat traps and seal the entry points in the facility. Center is on a monthly maintenance plan with the exterminator company. The staff ratio to children average 1:10 making supervision of children possible..The allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation are UNSUBSTANTIATED. Notice of Site Visit (LIC9213) was given. The notice shall be posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty. Exit interview conducted with Director, (LIC9213). A copy of the Appeal Rights (LIC 9058 FAS 01/16) were given and explained. Director's signature on this report acknowledges receipt of these rights. An LIC9102 technical violation was reviewed and issued.
