On October 16, 2025 at 11:00 AM, Licensing Program Analysts (LPAs) Brian Fung and Fernando Hernandez conducted an unannounced Annual inspection. LPAs met with licensee Cintia Veronica and advised the purpose of the inspection. Licensee provided LPAs a tour of the home inside and out. There were ten children in care at the time of the inspection and two adults in the home Licensee and assistant.

LPAs observed required Licensing required documents posted prominently at the entrance. Fire and earthquake drills are being documented every six months. Last drill was conducted on 10/15/2025. Fire extinguisher 2A10BC mounted on the wall inside the school room was last purchased 8/20/2025. Fire and carbon monoxide detectors were observed at 1:10 PM. Because children were sleeping at the time, licensee confirms the working condition of the fire and carbon monoxide and will send a video of the working detectors.

Children in care have access to a garage playroom, one restroom, and back yard. Licensee was reminded that a garage is not allowed to be used for childcare. The licensee was asked by LPAs to submit a updated LIC 279 form stating that she will be updating her facility to move her garage child care to the living room. Licensee will also provide to LPAs a new facility sketch. Licensee agreed and will move child care toys and furniture from garage to the living room by Monday 10/20/25 for children to use for child care. Licensee will also provided a declaration plan state how to ensure the garage will not be used moving forward. Sliding door located in the living room is broken and is to be fixed and licensee discussed that she will complete the replacement door by Monday 10/20/25. Licensee also state she will put a baby gate on the stairs to prevent children access to the second floor. A type B deficiency was cited.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The gate opens inwards towards the pool due to a short 1 feet high gardening wall. which poses/posed a potential health, safety or personal rights risk to persons in care.

The licensee will purchase a rescule pole with life hook, life ring, and pool alarm immediately and show proof with pictures taken of receipt and picture proof of placement of the missing pool safety items by 10/20/25. Moving forward, licensee wil also print and write down the daily pool inspection log and licensee will take a picture of the pool inspection log and send to LPA by 10/20/25.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. The garage was converted into a childcare room for play and sleep. However, there is not approval of a permit for the conversion of the garage from the city and fire department. A wall was installed to block the garage door. LPAs observed children eat, play, and sleep in the converted garage. which poses a potential health, safety or personal rights risk to persons in care.

Licensee will move all her toys and furniture from the garage to the living room. The licensee will also create an updated LIC 279 form along with an updated facility sketch. The licensee has provided the LIC279 form and facility sketch to LPA. Licensee will fix the broken slide door that leads to the back yard where the pool is located. The latched door to the children's child care living room is to be fixed with a new latch. Licensee will send picture proof of all corrections by 10/20/25.
Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.

No fireplace was located inside the house. Kitchen is off limits to children. Children have access to toys that are age-appropriate toys and activites inside and outside of the home. Back yard play area is enclosed and has plenty of bicycles, scooters, and activities. There are no guns and/or ammunition located on the premise. Upon inspection, a pool was found outside the other side of the back yard. The yard where the pool is seperated by a large wooden fence from the play back yard children uses. The pool also has a steel fence 6 feet high and gate was found to be open towards the pool. In front of the gate is a short 1 feet high gardening wall that prevents gate from opening fully outwards from the pool. LPAs discussed with licensee that the pool gate must open outwards the pool. Upon inspecting the pool, the pool is missing a daily pool inspection log, rescue pole with life hook, a life ring and pool alarm and/or pool cover. On 8/18/2025, LPA Fung and Licensee discussed about the new pool safety regulations. During inspection, the pool safety regulation items were not purchased or found upon inspection. The licensee discusses that there will be construction to remove the 1 foot tall gardening wall. Licensee will schedule to fix the pool gate after construction of the gardening wall is removed. Safe pool regulatory items, such as rescule pole, life ring, and pool alarm will be purchased immediately. During the tour, LPAs did not observe any hazards/toxins items accessible to children in care. A type B deficiency was cited for pool safety regulation.

LPA reviewed five out of five children files. All children files were current. Licensee CPR/first aid is valid through 7/20/2026. Licensee was reminded a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter certification AB1207 every two years at www.mandatedreporterca.com Licensee’s mandated reporter certification is valid through 3/03/2027.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPAs discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPAs also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPAs observed a safe sleep log being documented for infants in care every 15 minutes.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the LICENSEE Cintia Veronica, confirmed that there are no Registered Sex Offenders living in the facility and LPAs completed the RSO profile in FAS.
