On June 14, 2024 Licensing Program Analyst (LPA) Aaliyah Zendejas conducted a case management - incident inspection at the family child care home (FCCH) for the purpose of following up on an unusual incident report received by the department on May 31, 2024. The incident was regarding a child who obtained a lip injury outside of day care hours (C2) (under care and supervision of parent) which ended up looking infected as well as some alarming commentary that came from another child (C1) (same parent). Both children ended up getting disenrolled from the facility. LPA met with licensee Denisse Lucatero and toured the interior and exterior of the facility. At the time of the inspection there was 1 child under the care and supervision of the licensee.

Licensee told LPA that C1 was not potty trained when entering the facility, but licensee was trying to teach C1 how to be potty trained by putting them on the toilet every 1-2 hours and asking if they needed to go to the bathroom. C1 only went pee in the toilet, which caused C1 to become constipated according to mother of child. C1 also got scared when their shoe laces ripped and thought their father was going to hit them.
Licensee told LPA that the child in care (C2) had received the cut on the lip and was trying to hide it when entering the facility. Licensee then asked C2 where they got it from and if they remember. C2 said they do not remember where it came from but eventually said that they hit themselves on a piece of wood. Licensee asked C2 if their parents hit them and C2 stated that they don't but sometimes "papa hits mama." Child ended up going to doctor's office and a doctor note was received and was kept in the facility, but there was no indication of an infection on the doctor's note.

LPA reviewed doctors note and interviewed licensee regarding C1 and C2. LPA advised licensee to keep record of whenever diaper changes or restroom breaks occur within facility and to have it documented for future reference and stored within facility. LPA also advised licensee to follow the orders issued by a doctor, and to keep all injuries received by children in care noted and placed in their files.
During today’s inspection, no deficiencies were cited.
Exit interview and review of report was conducted with Licensee Denisse Lucatero. Notice of Site visit was provided and must remain posted for the next 30 days.
