On March 20, 2026 at12:30 AM Licensing Program Analysts (LPAs) Laura Carone and Brian Fung conducted an unannounced inspection to conclude investigation for the above allegations. LPAs met with Harshini Maddox, Administrator and Melinda Day, Administrative Assistant and explained the purpose of the visit. LPAs conducted a tour of the facility inside and outside with Harshini Maddox, Administrator and Melinda Day, Administrative Assistant. LPAs observed a total of 76 children under the care and supervision of 15 staff. There are 3 preschool and 3 infant classrooms. Child care center hours are Monday through Friday 6:30 AM to 6:00 PM.

LPAs interviewed staff, parents, and reviewed records. Parents interviewed reported being happy with the care and supervision their child receives at the center. LPA's found no evidence of the allegation that staff are not meeting a day care child's diapering needs. Teachers interviewed reported following a diapering procedure that is posted on the wall next to the changing table. During center tour, LPAs observed the

diaper changing areas and restrooms. Every time a diaper is changed it is logged in the Bright wheel App for parents to view. LPAs obtained a copy of the diapering procedures and viewed the Bight wheel App.

For the allegation of staff are not properly trained LPAs obtained documentation on teacher training completed throughout the year and a sign in sheets for training.

The staff has a daily cleaning and sanitizing schedule that is followed. A cleaning service comes to clean the classrooms every night. LPAs did not find supporting evidence for staff are not properly maintaining the facility. LPAs obtained a copy of cleaning schedule and the contract with the cleaning service.

Parents interviewed reported their child had hand foot and mouth disease. The center reported the outbreak to Community Care Licensing, Ventura County Public Health Communicable Disease Program and parents. The outbreak spread to all classrooms for a total of 15 children infected. There are several siblings enrolled at the center. The first case reported was on March 2, 2026 in a preschool classroom. The Administrative Assistant, Melinda Day worked with Public Health and followed their recommendations to contain the outbreak. The last reported case was on March 20, 2026. Hand foot and mouth disease is a highly contagious viral illness. The center staff followed appropriate steps to contain the outbreak. The allegation of staff did not keep the facility free from an outbreak is invalid.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.

No deficiencies cited today. A notice of site visit LIC9213 was given. The notice shall be posted for 30 consecutive days. Failure to maintain posting as required will result in a $100.00 civil penalty. Appeal rights LIC9058 given.
