This is a change of location, previous facility number 566215927.

On 02/20/2025, Licensing Program Analyst (LPA) Fernando Hernandez conducted an announced pre-licensing - Change of Location at the above-mentioned residence. LPA met with Applicant Livnat Eliyahu and informed them the purpose of the inspection. LPA in the company of the Applicant and husband of the applicant toured the interior and exterior of the home in its entirety. The applicant informed the LPA of the intention to maintain operating hours of a Family Childcare Home (FCCH) from 8:00 AM to 4:00 PM, Monday- Friday. The applicant also informed LPA of the intention to provide care for children 1 to 10 years of age. Applicant was informed changes in licensing hours and/or the ages of children supervised and cared for can be altered upon notifying CCLD in writing of the given modifications and/or changes. At the time of the inspection there were no children present. The applicant reports there are three (3) adults that live in the home and three (3) adults has received criminal record clearance. However, LPA informed applicant regarding (1) daughter requires a CRC and license will be pending until CRC is cleared.

Applicant requested a change of location 12/02/2024, for a Large Family Childcare Home License. The residence is a Two-story home. The first floor consists of 1 bathroom, kitchen, playroom, outdoor play yard, living room, dining room, outdoor backyard, and garage. The second floor consists of 2 bathrooms and 4 bedrooms which will be entirely inaccessible to children in care. The home's playroom, outdoor play yard and 1 bathroom will be used for childcare purposes while the remainder of the home will be excluded from care.

LPA observed the play area, outdoor play yard, and bedroom to be used for childcare purposes to be clean and orderly. LPA observed age-appropriate equipment in the playroom. LPA observed the bedroom to have napping equipment for children in day care. LPA observed the kitchen and rest of the home to be made inaccessible to children with a child gate. LPA observed the bathroom to be clean and free of potential hazards. LPA notes that the sharps are stored in a locked cabinet in the kitchen made inaccessible to the children in daycare. LPA notes that medication is stored in Applicants bedroom which has been made inaccessible to children by use of child safety gate below the stairs. LPA observed cleaning supplies to be stored in the garage which has been made inaccessible by use of a lock. Applicant informed LPA that they will be keeping other detergents and cleaning supplies in the laundry area which is stored in the garage made inaccessible. LPA observed an electric fireplace that does not generate heat however it is in an excluded part of the home, that is inaccessible as well.

LPA observed the outdoor play space in the backyard area which is segmented off from the pool and any other areas in the backyard that will be inaccessible to children in care. LPA notes that outdoor play space is completely enclosed by use of a fence, which is separate from the fence that surrounds pool. LPA observed a swimming pool which is enclosed with 5-foot fence, the gate is self-latching that opens away from the swimming pool. The gate is equipped with self-closing and self-latching mechanism. LPA notes pool alarm was observed and found operational. LPA notes approved life ring and 12ft rescue pole was observed. LPA informed applicant of the importance to perform a daily inspection of the safety equipment and maintain a log of the inspections to be provided to the department upon request. LPA observed age-appropriate equipment within the outdoor play yard. LPA observed the exit ways to be secured with latches that are beyond the reach of children in care. LPA observed plenty of shade and space for children in care. LPA observed a (2) storage sheds that will be used to store yard equipment is made inaccessible to children in daycare.

LPA observed a regulation fire extinguisher (2A10BC) that was serviced 09/10/2024. LPA reminded applicant to purchase or service a regulation fire extinguisher annually. LPA observed a combination smoke and carbon monoxide detector which was tested at 10:12AM and found to be operational. Applicant informed LPA that there are no firearms in the home. Applicant's Mandated Reporter training was completed on 08/04/2024, and CPR/First aid was completed on 03/19/2024. LPA informed applicant that it is their responsibility to ensure that their training and certificates are current. The Calabasas Fire Department granted a fire clearance following an inspection completed at residence 02/14/2025.

Applicant, was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Applicant provided proof of control of property. Applicant owns the home.

LPA reviewed with applicant the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

On this date, 02/20/2025, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

The home does not meet Title 22 Division 12 requirements of a Large FCCH license. Corrections are needed prior to licensing and will be pending LPM approval. These corrections are listed below. Together with the applicant the agreed time needed to complete corrections will be 03/07/2025.

1) Criminal Record Clearance for (1) Eilai Eliyahu, applicants daughter

A notice of site visit was given to applicant and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the applicant Livnat Eliyahu.
