Licensing Program Analyst (LPA) Angel Ascencio conducted a subsequent visit to the above facility to deliver findings. LPA Ascencio met with Administrator Saundra Jarmon at 8:00 a.m. Entrance interview conducted.

On 01/24/2022, the Department received a complaint alleging that staff is limiting showers for residents, staff limits hot water for residents, staff locks residents out of the facility, staff threatens residents, staff makes resident leave facility when sick, and staff limits restroom use for residents. On 01/27/2022 starting on 2:55 p.m. and on 07/07/2023 starting at 2:45 p.m., interview with Administrator Saundra Jarmon revealed that after attending day program, Resident #1 (R1) comes home and takes long showers. This would not be a problem, but we have five (5) people that live here that would like to shower as well. Two (2) or three (3) hours go by and R1 is still showering while their roommate is waiting their turn.

Administrator Jarmon stated there is not a limit on shower usage, not a limit on restroom usage and not a limit on hot water usage; however, there are other residents to account for. We have had meeting with the residents and talk to them about being courteous regarding shower usage and time in the shower. But I have not limited them or set rules for them. Same goes with the hot water. It is not a limit set for them to use the hot water, but because we are in a small home and 5 people live here, the hot water can be used up quickly. I just remind all residents not to use up all the hot water.

On 01/27/2022 starting at 03:50 p.m. and on 07/07/2023 starting at 3:46 p.m., interview with residents revealed that there is not a limit on the restroom usage, no limit on the showers and no limit to the hot water they can use. Additionally, residents stated when R1 was living at the home, there was problems with the hot water due to R1 taking longer than normal showers, up to 3 hours. Moreover, residents added that although R1 was the resident causing the problems regarding hot water, Administrator Jarmon never set rules or limits on the hot water. Additionally, residents confirmed that Administrator Jarmon had various meetings with all the resident to remind them to be courteous of the amount of time to be spend while showering and how many times we use the restroom. Lastly, resident interviews added that Administrator Jarmon has never told the residents not to shower, and not to use the restroom. Interview with R1 could not be conducted as R1 moved out of the facility on 01/22/2022; whereabout is unknown. Based on evidence gathered throughput the investigation, there is insufficient evidence to determine that staff is limiting showers for residents, staff limits hot water for residents and that staff limit restroom use for residents. Thus, the allegations are deemed unsubstantiated at this time.

Regarding the allegation, staff locking resident out of the facility. On 01/27/2022 starting on 2:55 p.m. and on 07/07/2023 starting at 2:45 p.m., interview with Administrator Saundra Jarmon revealed that the resident cannot be left unsupervised at the facility. The residents do not have a key to the facility, but I am always here in the afternoon when they get back. Further interview with Administrator Jarmon revealed that Monday – Friday, the resident participates in day program, leaving for that at 8:10 a.m. and returning to the home at approximately 3:00 p.m. In between their programming, Administrator Jarmon conducts her errands for the day making sure to return before 2:00 p.m. In addition, residents let me know where they are going and what time they are returning so I can be here ready to open the door for them. On 01/27/2022 starting at 03:50 p.m. and on 07/07/2023 starting at 3:46 p.m., interview with residents revealed that they do not have a key to the facility. If they want a key, they can talk to Administrator Jarmon to try and obtain a key, however there is no need since someone is always here to meet them.

Residents also stated the system they have in place is working and have not had any problems. The resident can freely come and go only letting Administrator Jarmon know their expected return time. Lastly, resident interviews added that they have never been locked out of the facility or have never been denied entry into the facility. Interview with R1 could not be conducted as R1 moved out of the facility on 01/22/2022; whereabout is unknown. Thus, based on interviews, the allegation of staff locking resident out of the facility is deemed unsubstantiated at this time.

Regarding the allegation, staff threatens residents. On 01/27/2022 starting on 2:55 p.m. and on 07/07/2023 starting at 2:45 p.m., interview with Administrator Saundra Jarmon revealed that R1 was living at the facility and would not follow house rules. R1 would smoke in their room, drink in their room and would arrive at the facility after hours, 11:00 p.m. or later. Administrator Jarmon added that although R1 was constantly breaking house rules, R1 was given multiple chances to do and be better. I tell my residents the house rules and if they don’t like it, they are welcome to move. Administrator Jarmon added that they never threatened R1 with an eviction, but informed R1 that due to R1 breaking house rules, it can lead to an eviction. Lastly, Administrator Jarmon added that because of the alcohol and drug use, R1 could have lost their placement at the home, in addition to the landlord evicting the entire home if they would have found out about the constant usage of drugs and alcohol. Interview with R1 could not be conducted as R1 moved out of the facility on 01/22/2022; whereabout is unknown. On 01/27/2022 starting at 03:50 p.m. and on 07/07/2023 starting at 3:46 p.m., interview with residents revealed that they have not been threatened by Administrator Jarmon in anyway. Residents revealed being happy and satisfied with what the home brings to them, in addition, being in a safe and comfortable environment, a place to call home. Based on evidence gathered, the allegation of staff threatens residents is deemed unsubstantiated at this time.

Lastly, regarding the allegation, staff make residents leave the facility when sick. On 01/27/2022 starting on 2:55 p.m. and on 07/07/2023 starting at 2:45 p.m., interview with Administrator Saundra Jarmon revealed that R1 left for day program and was picked up by their Case Worker shortly after. I was cleaning around the house and received a text message from R1’s Case worker indicating that they had picked up R1, were headed to the doctors, and that there were concerns, such as eating, not bathing, and fatigue with R1.

When the Case Worker and R1 came to the home, the concern was that I refused to let R1 come back to the home due to COVID-like symptoms. R1 and all the residents were immediately tested, and everyone came back negative. Administrator Jarmon added that the day program indicated R1 had shortness of breath, but R1 has a history of Asthma and allergies in addition to being a smoker.

Either way, all residents test came back negative. Even if they were positive, the resident here are in a private apartment except for two (2), they could have come here and I would have taken care of them. Interview with R1 could not be conducted as R1 moved out of the facility on 01/22/2022; whereabout is unknown. Interview with Case Worker on 02/08/2022, starting at 1:05 p.m., confirmed what Administrator Jarmon indicated but added that although R1 indicated that they have shortness of breath, Administrator Jarmon refused to let R1 go to the doctor or get an inhaler. On 01/27/2022 starting at 03:50 p.m. and on 07/07/2023 starting at 3:46 p.m., interview with residents revealed that they have never been asked to leave the facility when sick. In addition, interviews indicated that Administrator Jarmon keeps a close on them occasionally taking them to their doctors’ appointments and taking care of them at home if they are sick. Based on evidence gathered throughout the investigation, there is insufficient evidence to support the claim that staff makes residents leave the facility when sick. Thus, the allegations is deemed unsubstantiated at this time.

Exit interview conducted and a copy of the report was provided to Administrator.
