32 | BATHROOMS: There are four (4) full bathrooms and one (1) half bath. Three (3) of the full baths are designated for client use and one (1) is in the isolation room. The half bath is designated for staff and visitors. All bathrooms had functional fixtures and contained all required supplies. All bathrooms were supplied with appropriate paper and hygiene products. At 10:30AM, hot water temperature in Bedroom #1’s bathroom measured at 129.9 degrees F, which is above the required range of 105-120 F. Staff adjusted the water temperature and LPA remeasured at 11:52AM; water measured at 128.9 F. Staff readjusted and LPA measured a final time at 12:20PM; water measured at 113.4 F. Hot water temperature in the other client restrooms were measured within the required range. LPA observed faucets in bathrooms to contain warning stickers of temperatures exceeding 120 F.
LAUNDRY: Laundry Room is located on the entry level. Laundry and cleaning supplies were observed to be inside a locked hallway closet.
COMMON AREAS: These included two (2) Living Rooms, the Dining area, and an activity room. All common areas appeared to be clean and were appropriately furnished. There were no visible hazards. Fire extinguishers were observed to be fully charged and last serviced on 12/16/2025. Hardwired combination smoke and carbon monoxide detectors were tested at 10:44AM and were functional at the time of the visit.
TREATMENT/THERAPY & OFFICE AREAS: There is a group therapy room, an individual therapy room and an office designated for Staff use, which is used to store medications as well as first aid kit. The Offices are kept inaccessible to clients unless properly supervised.
GARAGE AND GROUNDS: The garage is used as an activity room/gym as well as to store emergency food and water and additional supplies. There is a front and back yard. The backyard is fenced with lawn and garden areas. The backyard has a patio with furniture appropriate for outdoor use and shade. There were no imminent or immediate visible hazards. Emergency passageways were clear of obstructions.
RECORD REVIEW: Staff and client files are stored electronically. Beginning at 10:48AM, LPA reviewed six (6) out of six (6) client files and four (4) staff files for documents including, but not limited to: admission agreements, health screening, TB test, staff training records, fingerprint clearance, needs and service appraisal, personal rights, and first aid/CPR training. All staff and client records reviewed were in compliance with Title 22 regulations at the time of the visit.
