This Case management visit was conducted to address the deficiencies noted during complaint control # 29-AS-20210512093307 investigation visit conducted on 5/24/21.

During facility tour on 5/24/21 starting at 11:03 AM with the Administrator LPA observed hair dye, 2 pairs of scissors and clorox toilet bowl cleaner in the salon, laundry detergent in the laundry room of the Maricopa building all accessible to residents. During facility tour at 11:36 am LPA observed a cart with a bucket of paint, tools and spackling in outside area accessible to residents. LPA reviewed resident records and obtained copies of pertinent documents.

Pursuant to Title 22 Division 6 Chapter 8 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 809-D):
