This Case management visit was conducted to address the deficiencies noted during complaint control # 29-AS-20210608163749, 29-AS-20210512093307, 29-AS-20210422132534 and 29-AS-20210401111205 investigation visits conducted on 6/15/21.

During facility tour on 6/15/21 starting at 10:16 am LPA's observed detergent and scissors in the unlocked laundry room of the Topa Topa building accessible residents. During facility tour at 10:24 am LPA's observed dish soap in the unlocked Salon room of the Maricopa building accessible to residents. During facility tour at 10:27 am LPA's observed Lysol disinfectant spray in unlocked resident room #105 in the Maricopa building accessible to residents. During facility tour starting at 10:41 LPA's observed a screwdriver, all purpose cleaner, disinfectant, glass and surface cleaner, carpet cleaner, lubricant and rug and room deodorizer in the staff bathroom accessible to residents.

Pursuant to Title 22 Division 6 Chapter 8 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 809-D):

Civil penalties assessed in the amount of $500.00.

Exit interview conducted, todays reports and civil penalties were reviewed and emailed to the Administrator.
