Licensing Program Analyst (LPA) Valeria Conway arrived at the facility unannounced to conduct a required annual visit at 11:00 A.M. The LPA met with Administrator Ramona (Mona) Keeton. LPA explained the reason for today's visit. Entrance interview conducted.

The facility is vendored through Tri-Counties Regional Center as a level 3 home. The Facility does handle cash resources for some of the clients, however at the time of the visit, no client requires this service. Surety Bond is current. No clients were present during today’s visit. The following was observed:

Beginning at 11:25 A.M., the LPA, along with Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards and the facility is in compliance with Title 22 Regulations.

Hardwired combination smoke alarms/carbon monoxide detectors were tested at 11:44 A.M. and functioned properly. The fire extinguisher was observed to be fully charged and last purchased on 09/21/2025. This facility is a two-story house, all six (6) bedrooms are on the second floor.

BEDROOMS: The facility consists of six (6) total bedrooms, four (4) of which are private client bedrooms and two (2) are designated for staff use. LPA observed all client bedrooms, which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

RESTROOMS: Two (2) restrooms are designated for shared client use and one (1) is designated as a staff restroom. Client restrooms are clean and sanitary and in operating condition. Hot water was checked in the client restrooms and measured within the required range. LPA observed a complete first aid kit under each sink.

KITCHEN: Kitchen appliances appeared to be in operable condition. The facility has a sufficient supply of perishable and non-perishable food and emergency dried food inside a pantry cabinet. LPA conducted a review of expiration dates on product labels. At 11:37 A.M. hot water measured at 110.3 degrees Fahrenheit.

COMMON SPACES: Living room, family room, and dining room furniture were observed to be in good condition. The LPA observed the required postings upon entry. The facility maintained a temperature of 71 degrees. Facility provides sufficient space to accommodate both indoor and outdoor activities. LPA observed a working phone available for residents use whenever needed. There is also an ample supply of linen, towels and paper products. LPA observed night-lights were present in the hallways.

OUTDOOR SPACE: The backyard has a covered outdoor area equipped with furniture for clients’ use. All exits and passageways were observed to be free of hazards. There were no bodies of water noted at the time of the visit. Facility has one side gate. LPA observed a side gate to be self-latching and closing with clear passageways for emergency exit use. LPA observed a locked shed; where lawn mower and gardening tools are stored.

Garage: The garage is attached to the house. Garage was observed to be locked. LPA observed a freezer and a fridge containing extra food for clients. Emergency water was observed in the garage.

LAUNDRY ROOM: Laundry room was observed to be locked and contained all chemical supplies, sharps and knives and clients and staff files.

CLIENT FILES: Beginning at 12:04 P.M., LPA reviewed all four (4) client files for, but not limited to: physician's report, proof of TB test, Admission Agreement, and cash resources. All client files reviewed were complete and contained all documentation required. However, the LPA requested an updated medical assessment for Resident #1. Technical assistance issued (TA)

STAFF FILES: Beginning at 12:53 P.M., LPA reviewed three (3) staff files for, but not limited to: fingerprint background clearance, health screening, TB test, and training records. All three (3) staff files reviewed were in compliance with regulation.

MEDICATION REVIEW: Beginning at 1:30 P.M., LPA reviewed medications for four (4) clients. LPA observed the medication securely stored in a locked safe located in the hallway near the entrance of the home. All medications reviewed were documented and stored in compliance with regulation.

LPA requested the following documents: Personnel Record LIC 500, Register of facility Clients/Residents LIC 9020, Liability Insurance and Surety Bond. Emergency disaster drills are conducted monthly, with the last drill conducted on 12/10/2025.

No citations issued. Exit interview conducted. A copy of the report was provided.
