13 | On 3/24/26, Licensing Program Analyst (LPA) Lauren Scott met with Director, Gwendolyn Adams to deliver the findings of the complaint investigation regarding the above allegations.
During the course of the investigation, LPA Scott conducted interviews and obtained information pertaining to the allegations. It was alleged that staff did not prevent children from being injured by other children and staff did not report incidents to parents or authorized representatives. Through interviews and documentation received, it was revealed that although some stated children injuries may have taken place at the facility, documentation was received and given to parents. It was also revealed that a plan was in place to prevent future injuries as well.
Based on the conflicting information obtained, the above allegations could not be substantiated or dismissed. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the findings are UNSUBSTANTIATED. A notice of site visit was provided and must remain posted for 30 days. |
