32 | It was alleged that staff did not prevent a day-care child from bullying another child in care. It was revealed through staff interviews that the facility is aware that Child 1 had been using verbal insults and physical violence towards other children in care. Interviews with children at the facility corroborate this allegation. The facility has reached out to Catalyst Kids Administration for support and has attempted to implement positive reinforcement, redirection, bonding activities, increased supervision or shadowing, and discussions with Child 1 about emotions. However, according to staff, parent, and children’s interviews, Child 1’s behaviors have continued.
Based on interviews, file review, and observations conducted the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Two Type-A Deficiencies are cited on a subsequent 9099-D page.
Upon receipt of Type A citations, the facility shall post and provide copies of the LIC 9099D for parents/guardians of children currently in care and for parents/guardians of newly enrolled children for the next 12 months. Facility must also keep the signed LIC 9224, Acknowledging Receipt of Licensing Reports LIC 9099D in each child's files.
An exit interview was conducted, and the report was reviewed with the Lead Teacher, Alex Lu.
