13 | Licensing Program Analyst (LPA) Erwin Tjhia met with facility representative, Irma Gonzalez to deliver findings of the complaint investigation regarding the above allegation. There were 6 children and 3 staff during the visit.
Throughout the investigation, LPA interviewed parents and staff. It was alleged that staff did not notify child's authorized representative of incident in a timely manner. Interviews with staff and record reviewed revealed that multiple phone calls were made to the child's authorized representative regarding the incident but were unsuccessful as the phone calls were not answered nor were returned. Interviewed with parents revealed that staff always communicated with them in timely manner when an incident occurred.
Based on the information obtained throughout the course of this investigation the above allegation, LPA Tjhia determined that the allegation was found to be UNSUBSTANTIATED, meaning although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.
Exit interview was conducted. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
