On February 24, 2023, Licensing Program Analyst (LPA’s) Corina Beckby and Lauren Scott and with Licensee, Leticia Gomez Vazquez, for the purpose of an unannounced annual inspection. There were 3 children present at the time of inspection. Licensee's operating hours are Monday through Friday from 7:30 AM. to 5:30 PM. Licensee has also applied to change from a Small Family Child Care Home to a Large Family Child Care Home. Fire clearance was granted by West Sacramento Fire Department on 2/9/2023. LPA verified the annual fees are current.

Licensee stated there are no new residents in the home since licensure. All adults subject to criminal background review have obtained criminal record clearance. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A health and safety inspection was conducted in all areas accessible to children and toured the one story, 3 bedroom, 2 bathroom home. Off-limits areas include 2 bedrooms (master and daughters), garage and shed in backyard. Off limit areas are being made inaccessible by closed locked doors. Licensee acknowledged that children may never enter these off-limit areas. LPA observed the required postings and a working phone and fire extinguisher. LPA observed smoke and carbon monoxide detectors, and verified they were both functional. LPA toured the kitchen area and verified knives were inaccessible to children in care. Licensee stated there are no weapons.

LPAs observed cleaning supplies properly stored in cabinet with baby proof locks, out of reach of children. LPAs observed medications are stored top cabinet in kitchen area out of reach of children. LPA observed living room area with age appropriate toys for children. The backyard is fenced, and there were no bodies of water on the property, there is also a locked shed in the backyard. LPA observed a trampoline in the backyard, and licensee stated she only uses it for children 5 and above and acknowledges she will use it in accordance with the manufacturer instructions. Licensee understands that prior to making alterations or additions to the home or grounds, the licensee shall notify the Department of the proposed changes.

3 Children's files were reviewed. Emergency information and required immunization records were on file. LPA observed a current roster and documentation that a fire drill is conducted at least once every six months. Licensee's immunization records are available in the facility file. Current EMSA pediatric CPR and First Aid certification was verified and expires on 6/29/2023 and Child Care Provider Mandated Reporter was discussed.

LPA’s discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage athttps://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPA discussed the requirement to check and log infant napping every 15 minutes, for infants under 24 months. LPA provided a copy of LIC 9227 Individual Sleeping Plan, for infants under 12 months.

This provider is not currently providing Incidental Medical Services (IMS) services to children in care. IMS policy was discussed. This provider is currently providing Incidental Medical Services (IMS) to children in care. LPA reviewed storage of medication, and child personnel records. An exit interview was conducted and report was reviewed with the licensee, Leticia Gomez Vazquez. A copy of this report will remain on file for a period of three years for public review upon request. Licensee's signature on this form acknowledges receipt of this form. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Effective today 02/24/2023, facility is approved for a Large Family Child Care License to serve 12 children (when there is an assistant present) with no more than 4 infants, or capacity of 14 children when 1 child is enrolled in Transitional Kindergarten or above and 1 child at least age 6 with a maximum of 3 infants. Without assistant, the ratios revert to those for small family childcare home.
