13 | On 6/2/26, Licensing Program Analyst (LPA) Lauren Scott and Licensing Program Manager (LPM) Chayntel Hunter, met with facility representative, Katrina Hopkins, to conduct a complaint investigation.
During the course of the investigation, LPA Scott conducted interviews, and obtained information pertaining to allegation. It was alleged that staff did not notify day care child's responsible party of incident in a timely manner. Interviews revealed that the staff did tell the child's parents of the incident within 24hrs but not on the day of the incident, nor was a written incdent report given that day. Facility Representative have trained staff to notify parents the same day of the incident.
Based on the information obtained throughout the course of this investigation the above allegations could not be substantiated or dismissed.Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the finding is UNSUBSTANTIATED.
Exit interview was conducted. A notice of site visit was provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
