25 | Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced to conduct a Required - 1 Year Inspection at approximately 9:10 AM, and met with Administrator Diana Paz. The inspection was focused on the Infection Control procedures and practices of this facility.
Before entry at central entry point, LPA/visitors required to fill out a COVID-19 questionnaire. Staff then asked LPA to sign-in on visitor sheet and have temperature taken. LPA then conducted walk through of the facility with Administrator and observed COVID postings throughout. Facility was a comfortable temperature of 72 F and exits were free from obstructions. Residents were clean and appropriately dressed. Mitigation plan was approved on 7/19/21 by Community Care Licensing (CCL).
Hand sanitizer is kept throughout the facility. Per Administrator and observation, updated infection control guidelines and PINs are communicated to responsible parties through an on-site information resource binder at Staff Information Center. High touch surface areas are disinfected daily. Residents could isolate in their own rooms if they became ill. LPA confirmed PPE and supplies to support a resident in isolation. All staff are fully vaccinated and boosted and 85% of residents are fully vaccinated.
Residents' emergency contact information has been updated and Administrator confirmed staff are familiar with 911 procedures and protocols. Toxins are secured and inaccessible in locked hallway closet, locked kitchen cabinet, and locked laundry room. Medications are centrally stored in a locked office and med. cart, making them inaccessible to residents. Last fire drill was performed in May. There were 3 Fire Extinguishers last serviced on 10/07/21.
