Licensing Program Analyst (LPA) Diana Campos met with Head Teacher Nova Ray for the purpose of conducting an UNANNOUNCED FIVE YEAR REQUIRED INSPECTION. Present during this inspection were 10 staff and 24 preschoolers in care. The facility was toured to conduct a Health and Safety Inspection.

There are ample age appropriate toys that appear to be safe and in good condition. The furniture and equipment is in safe condition and is free from sharp, loose or pointed parts. There are no standing bodies of water. All hazardous materials and toxins are kept out of the reach of children. One toilet seat in room B is not operable, the toilet seat has a small chair over it to prevent children from accessing. All surfaces accessible to children is clean and toxic free. OUSD preschool facilities have merged to an electronic Sign in/out verification system and not all parents/guardians are able to access upon drop off off/pick up. Menus were not posted but are available to parents via the Titan application online. The playground equipment is in safe condition and free from sharp, loose or pointed parts and the areas around or under high climbing equipment has appropriate cushioned material that absorbs a fall. Uncontaminated drinking water is provided both indoors and outdoors. All staff subjected to criminal review have been cleared through OUSD. Staff files were not reviewed today. As a condition of employment with OUSD all staff undergo a criminal background and health screening as well as verification of education with the appropriate credits prior to hire. Staff certification in CPR and First Aid is current and valid for opening and closing staff members at this site. A sample of children's files were reviewed and found to be complete. Medication in room A and B was properly stored and unexpired.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Head Teacher was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA also informed licensee Head Teacher of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with
