On July 29, 2022 approximately at 8:45AM Licensing Program Analyst (LPA) Kelly Phan arrived for an unannounced Required-1 Year inspection, and met with Assistant Director Hue Huynh. There were 123 children (7 toddlers and 116 preschoolers) and 23 fingerprint cleared staff members present during the inspection. The facility also has a toddler option between the ages of 18 months to 36 months in Daisy classroom. The facility is in ratio today. The facility was toured for a health and safety inspection. Hours of operation are Monday through Friday, 8:00am to 5:30pm.

CLASSROOMS: The ten (10) classrooms were inspected (Lily, Lotus, Magnolia, Orchid, Rose, Daffodil, Lavender, Tulip, Daisy, and Jasmine) . There are adequate play and learning materials available. The floors, furniture, and equipment are age appropriate and in good repair. There is adequate heating/air conditioning, ventilation and lighting. Drinking water is available inside and outside of the center. There is proper individual storage space for each child. The isolation area for sick children is both sides of the office, away from other children in care. The center has smoke detectors, carbon monoxides, working telephones, and fire extinguishers around the entire facility, which is checked twice a year by Bay Alarm.

BATHROOMS AND TOILETING AREAS: There are two staff bathrooms that is separate from the children's bathroom. Each classroom has sinks and faucets are in safe and sanitary operating condition. The children can reach the sinks and toilets. Supplies are available to the children. There is a sink and toilet inside the classroom that is temporarily shut off due to maintenance; per Ms. Hue they have scheduled appointment for someone to check on it next week during their staff development day (next Friday).

FOOD SERVICE AREAS: This facility provide AM and PM snacks with a vegetarian option for the children. There are weekly menus posted at the facility and lunch is provided by the center to children in care and only serves water or low fat milk and occasional juice during a child's birthday provided by the parents. Menus are rotated every week. All storage containers for solid waste have tight fitting covers and has no hazardous items. SEE LIC 809 C

OUTDOOR PLAY AREAS: The facility utilizes three play areas that has multiple play structures that have cushioning to absorb falls and that is anchored for stability and are physically separated from each other. per Hue, each classroom has 30 minutes to utilize outdoor area in both in morning and afternoon. There's also trees that provides shade to children while at play; per Hue children may stay indoors if the weather is too hot to play. There are also water fountains to be used outdoors. LPA observed there is small part of the gate that needs fixing as it is bent as result of children leaning or playing on it; per Hue they have already set up an appointment to have someone come out to provide them with an estimate to fix it.

RECORDS: All individuals subject to criminal record review have a clearance or exemption and have been associated to the facility. LPA reviewed 11 children's files and 9 staff files were reviewed at 11:00AM. All staff files have required health screening and Employee Rights and all children files contain Identification & Emergency, Personal Rights, and Medical Consent forms. Per Hue, all staff who opens and closes have a valid CPR and First aid on site. LPA reviewed the facility roster and obtained a copy. Mandated Reporter Training was discussed, and certificates were reviewed. The center is in compliance with the sign in and out procedure as there is a waiver for them to use a computerized system to capture parent's initials . Disaster drills are being conducted at least once every 6 months and the last one conducted was on 06/20/22. All required documents are posted in a public accessible area.

HEALTH RELATED SERVICES: There is IMS being provided at this time to three specific staff member (director and two assistant directors) to ensure medications are given safety and correctly to children in care, along with an allergy list in each classroom. All medication are stored at the facility office are not expired and are in original containers with a consent form from child's parent. The center is equipped with a fully stocked first aid kit that is available in the classroom and kitchen area.

There were no deficiencies were cited for today's inspection

Appeal rights and a notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00. Exit interview conducted and report was reviewed with Assistant Director Hue Huynh

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.


Incidental Medical Services (IMS) policy was discussed. This facility provides IMS to children in care. The facility is following and have developed IMS plan on file. When any changes to the IMS plan is made, an updated Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.


Licensee was reminded that California Law requires licensed Child Care Centers to report unusual incidents or injuries to children in care to child's parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or electronic mail. LPA informed the Facility Representative that all forms can be downloaded at www.ccld.ca.gov and encouraged the Facility Representative to email childcareadvocatesprogram@dss.ca.gov to be included in the Child Care Quarterly Updates distribution list. Licensee was also reminded that Mandated Reporter Training ("General" and "Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting www.mandatedreporterca.com.
