On 03/22/2023 at 10:00am, Licensing Program Analyst (LPA) Christina Uribe and Licensing Program Manager (LPM) Chandra Charles conducted an informal meeting at the Community Care Licensing, Child Care Program, Oakland Regional Office and met with site director, Sucheta Roy. Also present during the meeting are directors Hue Huynh & Maggie Baltazar.

The purpose of today's meeting is to discuss an incident which took place on 01/23/2023 where a 2-year-old student left the facility unattended. An investigation regarding the incident was opened and performed by LPA Uribe in which the facility was found liable and the incident was found to be substantiated. The facility was issued a Type A Violation and an immediate civil penalty of $500.00 on 02/09/2023. Due to the nature, the incident is being discussed today and video footage from the facility's security cameras are being reviewed. LPM Charles read aloud the complaint allegation details. Additionally, it was discussed how the facility has adapted to the new safety protocols that have since been put into place.

The facility representative, Maggie, stated that the changes that have been have been successfully put into place, the staff are following the new protocols accordingly, and that the parents of the children enrolled have expressed satisfaction and more security with these new protocols in place.

LPM Charles advised the facility representatives to hold a staff meeting, show the security footage of the incident and review the expectations of the staff to act appropriately when something out of place occurs, and to review the training video from the CCLD website "Supervising Children in Child Care Centers". It was also suggested that the on-board training for new staff members include these as well.

Report was reviewed with and copy was given to Sucheta Roy, Maggie Baltazar, & Hue Huynh.
