|(3) An annual random visit was made to the facility by LPA, Laura Chavez. This is a combination center with a separate preschool license. The facility file was reviewed prior to this visit. This is a Title 5 Program. Staff records indicate that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances. The facility operates Monday - Friday; 7:30am - 4:00pm. The facility was toured inside and outside. The floor and yard sketch previously submitted were reviewed. The infant and toddler classrooms appeared clean, safe, sanitary and orderly. The items which could pose a danger to children (detergents, cleaning compounds, and medications) were stored out of the reach of children. Poisons are locked in a janitorial closet. There are working fire extinguishers, smoke detectors, and carbon monoxide detectors located in each classroom. The toys, floors and other equipment appeared clean, safe, and age appropriate for infants. There is sufficient napping equipment. The changing table is within arm’s reach of a sink. A current menu was posted. Food prep areas are clean. Food is properly stored. Bottles and food containers brought from home are properly labeled. Garbage cans containing solid waste have tight fitting lids. The infant and toddler playgrounds are completely fenced and free of hazards. The infant and toddler playgrounds are physically separate. The playground equipment appeared in safe condition. There is wood chip and rubber cushioning underneath climbing structures and/or play equipment to absorb falls. There were no bodies of water observed. Records were reviewed. The sign in/out procedure was reviewed and in compliance. Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Children and staff records were reviewed. At least one staff member present possessed current CPR and First Aid certifications.
