Licensing Program Analyst (LPA), Marina Pilossian conducted an unannounced complaint visit for the purpose of concluding the investigation for the aforementioned allegations. LPA met with Laurie Tweedley, assistant Director. LPA and assistant Director toured the facility inside and outside on 6/7/19 at 11:00am. LPA observed 103 pre school age children, with 17 staff members including the assistant director Ms. Laurie Tweedley providing care and supervision.

The investigation of the above allegations consisted of interviews with staff, children and parents. LPA reviewed files. The information obtained revealed that staff #1, and #2 sometimes would yell, withhold water/food, physically pick up child/restraine child, handled child in a forceful manner, and humiliated a child.

Based on the information obtained from the interviews conducted by the LPA, the above allegations are substantiated. A finding that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standard has been met.

Personal Rights. Each child shall be free from corporal or unusual punishment, humiliation, intimidation, ridicule, coercion, threat, mental abuse, or other actions of a punitive nature. Director/licensee and all staff members to conduct a meeting/training to discuss how to protect the personal rights of children. Director shall submit a copy of the training agenda with the names of the staff that attended no later than 6/7/19.

restrain child by picking up and isolating from a scheduled activity, handled child in a forcefull manner, and humiliated child(ren). It was not intended toward a particular child. It was towards different children. This is a Type A deficiency as it posses immediate hazard to the health and safety of children in care.

Facility was cited Type A, violation for Personal Rights. Please see complaint investigation report LIC9099-D for deficiency cited.

Upon receipt of this report, the report must be posted along with the notice of site visit for 30 days for parents to view. Licensee must inform the parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months via form LIC 9224 Acknowledgement of Receipt of Licensing Reports.

An exit interview was conducted, appeal rights discussed, and a copy of this report was provided to the assistant Director during the visit.
