Licensing Program Analyst (LPA) Ann Dumolt met initially with Ms. De La Cruz for the purpose of a Health and Safety inspection. Ms. Lam arrived 20 minutes into the visit. Head Start is located on the campus of Sanchez Elementary School in a portable on the west end of the campus across from the Kindergarten. Per school district policy, there are no weapons or firearms stored on the premises. Visitors to the campus must sign in at the elementary office. Parents enter from a gate near the portable which is then locked during the morning (8 -11:30 a.m) and afternoon (12:30 to 4 p.m.) sessions.

Classrooms is equipped with age appropriate furniture, fixtures, equipment, and supplies. There are areas offering a variety of activities for the children to develop and master educational skills and activities of daily living. Restrooms are clean, odor free, and well supplied with toilet paper, paper towel, and dispensable soap. Staff visually supervise and are available to children who may require assistance with toileting. Handwashing is emphasized after use of restroom as when transitioning from outdoor play back into the classroom, and before and after meals and snacks. Instructions / reminders for handwashing are posted in English and in Chinese. There is a water fixture on one of the sinks for children to drink water. Flooring and carpeting are clean. Staff observed interacting with children in their classroom and play yard activities and are mindful of children's personal rights: Speaking to children in a calm voice at eye level and readily available to assist the children. Teacher child ratios are met under Title 5: 1 teacher for 24 children with 1 adult for every 8 children. On this visit 16 children were being supervised by a teacher and teacher assistant. Sign in and sign /out sheets reflect the number of children in care and census is within the licensed capacity of 18. It was noted that parents use full signature when signing children in or out.

Disinfectants are stored in an overhead cabinet out of the reach of children. Medications are stored in an overhead cabinet within locked boxes. Medications are within expiration dates and have protocols for their use. First aid supplies stored next to medication boxes and were inventoried. First aid supplies appear to be sufficient to meet the needs of children in care.

Snacks and meals are delivered from the school cafeteria. Meals and snacks are brought in containers to keep food warm. Perishables such as milk are stored in the classroom refrigerator. Countertop and flooring where meals and snacks are distributed are clean and free of vermin (bug casings or droppings, rodent droppings or urine). Trash can with tight fitting lid is used for disposing food wash and disposable dishware and utensils. Carbon Monoxide detector is mounted on the south wall of classroom near Smart board.

Yard is fenced and gated. Recent change includes fencing off a section of the yard on the west side of the Head Start portable. This allows Head Start a place to store its trikes and the trash used for food waste. Rest of play yard is shared by waiver with the 2 Kindergartner classrooms. The two play structures are age appropriate. Tot turf ("pour and play") is used as cushioning material under play structures and beyond fall zones. Paved area allows space for riding trikes. Grassy area is well groomed and free of pollinating weeds. Play structures and trikes are free of pointed, sharp, or loose parts. Covered patio and patio roof over one of the play structures offer shade. Water fountain is available for children to drink water.

Copy of staff records showing education units and current certification will be mailed out to LPA Dumolt. It is understood that education background, background clearances, and current certification in pediatric CPR and first aid are a condition of employment with the school district.. A sampling of children's emergency cared reviewed for parent contact information, who may sign child out, and consent for medical treatment. Required forms are posted: License, Personal rights, Parents Rights poster for Centers, Car Seat Law, and Emergency Disaster Plan are posted.

No deficiencies observed.

Exit interview conducted with Becky Lam. Copy of report and appeal rights given. Notice of site visit posted on door used by parents (door closest to gate) to drop off and pick up children. It is to remain on the door for 30 days or until the last day of school (13 June).
