Licensing Program Analyst (LPA) Doris Whitmore met with Director Wendy Bell for the purpose of a 3 Year Required Inspection. LPA Whitmore toured the facility, all identified rooms per facility sketch were inspected, the following was observed. There are two different schedules Pre- school hours are 8:40a.m. to 2:10 p.m..and 8:40a.m. to 3:30pm Monday through Friday.

Upon arriving at the preschool LPA observed children being dropped off by parents. LPA observed sign/sign out sheets in each classroom. Facility is operating within capacity limitations. LPA observed 4 classrooms for instruction. LPA observed Blue Classroom (ages 2 to 3 year years old) with a ratio of 2 teachers to 10 children, Yellow Classroom (ages 4-5 years old) with a ratio of 2 teachers to 10 children, Red Classroom (ages 3-4 years old) with a ratio of 2 teachers to 8 children and Orange Classroom ( ages 2 to 3 years old) with a ratio of 2 teachers to 8 children. A walk through of each was conducted, classrooms were found to be clean and free from any potential hazards. LPA observed classrooms to have dramatic play, arts/crafts, reading and learning activities. LPA observed wall heaters covered for safety. Each classroom is equipped with a first aid kit that travels with the class.

Furniture was found to be in good repair and age appropriate. There is adequate heating, lighting and ventilation in each classroom. Drinking water is readily available in each classroom. Director states that the isolation area for sick students is located in the Director's office. Program does not have a nap time for children. Preschool program has two program times.

The bathroom area(s) were inspected, there are sufficient toilets and sinks to accommodate the campus capacity. Toilets flush properly, toilet and sinks are reachable by the children in care. Each restroom has adequate Toilet paper and paper towels available. Bathrooms were found to be clean. There is adequate lighting/ventilation in the bathroom area(s)

The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm. The ADA requires that childcare providers not discriminate against persons with disabilities on the basis of disability, that is, that they provide children and parents with disabilities with an equal opportunity to participate in the child care center's programs and services.

Requirements for fingerprint clearances and associations were discussed with the licensee. Licensee can be cited a civil penalty of $100 per day, up to $500.00 (5 days) for the 1st offense and up to $3000.00 for the 2nd offense within a 12 month period. The licensee was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and licensing within the time frame specified by the regulation. The "Notification of Parent's Rights" (PUB394) poster must be posted in an area accessible to parents. The information regarding new legislation with regards to exemptions and Parent’s Rights was also discussed.

The licensee was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If a serious violation is cited, a copy of the licensing report (LIC809 or LIC9099) must also be posted for 30 days. If these requirements are not met, civil penalties in the amount of $100 per violation will be assessed. Copies of the reports must also be provided to each parent when a serious deficiency, Type A, is cited (LIC9224).
AThe licensee was informed of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on child care licensing, and provide many other helpful resources to the licensees and the public. Senate Bill 792: this bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles. LPA discussed the influenza waiver during the visit. New Immunization Requirement: Law enacted by SB 277, beginning January 1, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into child care or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.

Update on Incidental Medical Services: Facilities that provide Incidental Medical Services (IMS) must identify those services in their facility’s Plan of Operation and submit an updated Plan of Operation to the Department. Incidental Medical Services Include: Blood-Glucose Monitoring for Diabetic Children, Administering Inhaled Medication, Administering EpiPen Jr. and EpiPen or other Epinephrine Auto-Injectors, Glucagon Administration, Gastrostomy Tube Care (G-tube care), Insulin Injections Administration, Anti-Seizure Administration, and Emptying an Ileostomy Bag.

This facility provides Incidental Medical Services – IMS. Currently there are No children on medication Licensee shared with LPA the process. Lunches are not prepared by the school; Parents provide snack for their children. Cleaning supplies are out of reach to the children and is stored separately in a locked room outside of classroom.



Inspection of the outdoor play area was conducted climbing structures, swings, slides and other large play equipment is found to be securely anchored with adequate resilient cushioning material underneath and around the perimeter. Drinking water is readily available on the play yard. There is adequate shade for the children in care. Fencing around the perimeter of the play area is at least 4 feet high. Playground is free from miscellaneous debris such as tree branches, trash, leaves, etc.
Children and staff records were reviewed for completeness records were found to be complete.
Director advised that records for all children and staff must be maintained for 3 years after separation from the facility. A record of teachers/assistant’s names was check against current facility roster; Staff are currently associated to the campus in which they are assigned to work. LPA observed CPR/First Aid expires 10/02/2022 Mandated Reporter Certificate Completion Date 08/26/2022. LPA advised Director that staff who has currently been working for the school for a long period of time forms should be updated. LPA shared the Regulation 101216.The required fire extinguisher, smoke detectors and carbon monoxide detector are in operable condition. LPA observed required postings in the facility (Facility License, Emergency Disaster Plan, Parents Rights Poster). Licensee was reminded it is the Licensee’s responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised on how to access quarterly reports, forms, and regulations for Child Care online at www.ccld.ca.gov. Licensee was also encouraged to read the Child Care quarterly updates every season as the come out to stay informed of any changes or updates to the regulations. Licensing must have the facility’s phone number at all times; if the phone number is changed, licensing must be notified. Regulation prohibits the smoking of tobacco on the premises.

The licensee was informed of the responsibility to report suspected Child Abuse by calling the Child Abuse Hot line at 1-800-540-4000. Also call the CCL office within 24 hours of the Unusual Incident and follow up with a written Unusual Incident/Injury Report (LIC 624B) within 7 business days.

Mandated Reporter: Beginning on January 1, 2018, AB 1207, requires all licensed providers, applicants, directors and employees to complete General and Child Care Mandated Reporter training as specified on their mandated reporter duties and to renew their training every two years.
