At 9:25 AM Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced required 3 year inspection. LPA met with Principal Barbara Walker, who guided the LPA on a tour of the facility. This program consists of 5 classrooms: RM1 (A and B), RM2, RM3, RM 4 and RM 5. Facility operation hours are Monday through Friday from 7:00 AM to 5:30 PM.

All areas identified on the Facility Sketch were inspected. Upon arrival, the following was observed: Room # 1A and 1B were combined and observed to be out of ratio with 1 Teacher, 2 Aides and 1 Volunteer with 25 children. Room 4 and 5 were combined and observed to be out of ratio with 1 Teacher and 3 Aides with 25 children. LPA advised that 1 teacher and 2 Aides can have up to 24 children- and aides need to be directly supervised by a qualified teacher at all times.
Room # 3 was observed in ratio with 1 Teacher ( Principal), 2 Aides and 17 children and Room # 2 was observed in ratio with 1 Teacher 2 Aides and 1 Volunteer with 14 children. Staff names were recorded.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. Children have their own cubby to store their belongings. Linens are washed weekly by the school district. Napping equipment (cots) were observed in designated storage area in each classroom. Per Principal, the isolation area is located in the staff lounge. In the staff lounge a mat can be placed on the floor for an ill child and the staff restroom is readily available if an ill child requires it. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. LPA observed that the facility does have a carbon monoxide detector which was tested, and it is in operable condition.. ------------ pg. 1 of 5 --------------

General sanitation was observed. Availability of indoor drinking water was observed in each classroom. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children in all areas.

All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids.

Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed.

Children's roster was reviewed and is current. Disaster drill log was available, last drill was conducted on May 23rd, 2024.

Menus are posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Lunch was reviewed for availability, quantity and appropriateness to children in care. Preschoolers are provided with breakfast, lunch and pm snack by the Los Angeles Unified School District.

All individuals present have obtained a criminal record clearance or criminal record exemption. All staff have received an active criminal record clearance as a condition of their employment with the Los Angeles Unified School District. There is at least one person trained in CPR and Pediatric First Aid present during this inspection.

AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. Website provided: https://www.mandatedreporterca.com/training/child-care-providers. At 11:00 am LPA observed that Staff # 5 has an expired Mandated Reporter certificate. Certificate for staff # 5 was provided after care tool was closed and citation was given.

LPA did not review complete staff files during the inspection due to the staff files being stored at the administrative office. LPA Lopez will review staff files at a later time and date. If staff files were to be incomplete during that time, a case management inspection will occur at the facility to reflect the incomplete files.

Children’s Records were reviewed for completeness; Inspection of required forms was made. In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.

LPAs issued the Children’s Record Review (LIC 857) to the Principal during this inspection.

At 10:30 am LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA observed that Child # 1 has expired medication in the facility. Per Principal child is no longer enrolled and will call parents to pick up medication.

LPA discussed the safe sleep regulations with licensee facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Facility representative was informed of the MyChildCarePlan.org website; a
consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Based on the LPA’s observations and records reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code od Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.

LPA Roxana Lopez informed facility representative Barbara Walker that this report dated 7/9/2024 document(s) 1 Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Roxana Lopez informed the facility representative to provide a copy of this licensing report dated 7/9/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days.
