On 10/05/2023 at 1:00 p.m., Licensing Program Analyst (LPA) Cristina Castellanos and Judy Laureano conducted an unannounced Annual Required Inspection at 14321 Grevillea Avenue, Lawndale, CA 90260 and met with Licensee, Maria Guadalupe Pallares. Present during the inspection were licensee Pallares and assistant E. Haro. LPAs disclosed the purpose of the inspection and were granted entry into the home by the licensee. The purpose of this inspection is to ensure that health, safety and personal rights as required by Title 22 Regulations governing California Child Care Homes will be met by the licensee. The facility is licensed for a Large Family Child Care with a max capacity of 14 children. Currently the facility is available to take children ages 9 months to 5 years old. The facility hours of operation are between the hours of 8:00 a.m. – 5:00 p.m., Monday - Friday. Licensee is not available for evening, overnight care, or weekend care. Currently living in the home is licensee, licensee’s husband L. Carrascosa and adult son.

This is a single-family home with 4 bedrooms, 2 bathrooms with living room/dining room area, kitchen, and laundry room. LPAs observed a detached garage in the back. Childcare is conducted in the living room and two front bedrooms. Per licensee, the living room is use for eating and occasional extra activity space. The bathroom that children use is located next to bedroom number two.

Licensee confirmed the following areas are off-limits to children in care, kitchen, laundry room, 3rd and 4th bedrooms and the 2nd bathroom. The detached garage and backyard were confirmed to be off-limits. LPAs observed a safety gate between kitchen and living room, making the rest of the home inaccessible to the children in care. LPAs reminded licensee that any areas designated as off-limits should be made inaccessible during the hours of operation and/or while children are present.

LPAs observed two pets, small dogs that are kept away from the children in care. Licensee confirmed that the dogs are kept in the off-limits area in the kitchen/laundry.

The following was also observed by LPA during inspection:
§ All adults living in the home and assistant and/or substitute care providers have submitted fingerprints and child abuse index check forms to Department of Justice and received a California clearance or exemption.
§ Home is neat and clean. Adequate heating and ventilation for safety and comfort were observed in the space.
§ No fireplaces and/or heaters were observed.
§ Home has a fully charged fire extinguisher which is at least a 2A:10BC.
§ Home has a working smoke alarm and a carbon monoxide detector.
§ Home has a working telephone.
§ All poisons are locked. LPAs advised licensee to add an additional safety latch for added precautions.
§ Hazardous materials are inaccessible to the children in care:
Kitchen: all sharp utensils and cutlery, cleaning supplies, medicines, drawers and cabinets with liquor, plastic bags, and sharp things or small things children can swallow, etc.
Bathroom: shampoo, mouthwash, toothpaste, medicines, perfumes/lotions/cosmetics, solvents, etc.
§ Licensee confirmed the home provides meals and snacks. LPA discussed the importance of maintaining a system where allergies and food restrictions are noted. Licensee confirmed that home is currently participating with a local food program - Evergreen.

§ Licensee confirmed there are no firearms or weapons in the home. None were observed by the LPAs during inspection.


§ Outdoor play area is free from defects or dangerous conditions. LPAs observed play equipment to be age-appropriate toys
§ Front yard is use for outdoor play area which is fenced.
§ Home does not have a swimming pool, spa, hot tub, fishpond, or any other bodies of water.
§ Toys and playthings are safe, clean, and appropriate for the age of the children.
§ Baby-walkers, bouncers, jumpers, and similar items will not be used for children in care and are kept.

Licensee ensures that children in care are supervised at all times and is aware children shall not be left in parked vehicles. Car seats are used for transportation purposes only and are not used for sleeping children.

Capacity as specified on the license is being maintained during today’s inspection. LPAs observed 9 children present during today’s inspection.

Licensee’s Pediatric CPR and Pediatric First Aid was taken on 07/15/2023. LPAs reminded licensee the importance of making sure all vendors providing Pediatric CPR and Pediatric First Aid need to be EMSA approved. LPAs discussed all necessary forms needed in each staff file and children’s file. LPAs provided licensees with a current copy of the LIC 311D to use as a reference when auditing files. LPAs reviewed 6 children’s files and observed files to be complete.

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPAs discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA reviewed with licensee the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the licensee.

Licensee, Pallares was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child-care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.
