On 10/29/24 Licensing Program Analysts (LPAs) Portia Bowden and Jeanette Estrada, conducted an unannounced Annual Required Inspection at the above facility. At 11:30AM LPAs met with Licensee, Mellony Patterson, explained the purpose for our visit, provided LIC 126 (Entrance Checklist, and were guided on a tour of the home. Per Licensee Hours of operation are 23 hours a day 6am-5am Monday-Sunday. LPAs reminded Licensee that hours of care cannot exceed the maximum of 23 hours per day. During tour, LPAs observed 4 children in care and 2 adults present. Per Licensee there are 22 enrolled on a staggered schedule. Per Licensee she provides overnight care as needed as well as transportation to and from school. LPAs discussed with licensee supervision and safety when providing transportation and overnight care services.

Individuals residing in the home have been discussed and noted. All adults present in the home have obtained a criminal record clearance or exemption. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. The following was observed and reviewed during this inspection:



This is a one - story home consisting of 4 bedrooms and 3 bathroom. Areas used by the children include the day care room, kitchenette in the day care room, 1 bathroom located next to day care room, and enclosed gated front yard. Per Licensee areas off limits to children include: all 4 bedrooms, living room, kitchen and backyard. Per licensee, the daycare room will be used as an isolation area with sick child being separated from other children in care until being picked up.
LPA reviewed required posted documentation for Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. Facility records were reviewed for LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan and Disaster drill log, last drill conducted on 9/26/24

Per licensee All poisons are kept in a locked storage area in the off limit backyard area. Detergents, cleaning compounds and other hazardous items are kept in a locked storage shed in the front yard area and inaccessible to children in care. Per licensee there are no firearms or bodies of water on the premises. LPAs observed a working combination smoke detector/carbon monoxide detector in the hallway directly next to the daycare room. During inspection, LPAs observed 210ABC fire extinguisher fully charged with service tag date of 10/15/24

During inspection, LPAs observed age appropriate toys and play equipment in adequate condition. The home has a working telephone service via Licensee’s Cell phone. The outdoor play area in the front yard is completely fenced with no hazards to children present. LPAs observed age appropriate toys in good condition in the outdoor area. Per Licensee they also use nearby park for outdoor play. LPAs observed kitchen to be clean and free of clutter or vermin. Per licensee she provides breakfast, lunch, dinner and morning/afternoon snack.

LPAs reviewed 5 Children’s files and found 3/5 to be complete. 2 out 5 children’s files were missing immunization records. Per Licensee she has been actively requesting immunizations from parents. A technical violation will be issued.

A review of staff records indicates that licensee has updated mandated reporter certificate with expiration date of 1/2025 and updated immunization records on file for measles, pertussis, influenza and TB clearance. During staff record review, LPAs observed pediatric CPR/First Aid certificate with expiration date of 10/2025.



During inspection all children were observed to be treated with dignity and respect, they were observed to be receiving safe, healthful and comfortable accommodations, furnishings and equipment, and free from corporal and/or unusual punishment.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: https://www.ada.gov/resources/child-care-centers/.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During Exit Interview, Licensee Melloney Patterson, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

1 Technical Violation will be issued for immunizations.

Exit interview conducted with Licensee Melloney Patterson and report provided. A notice of Site visit was provided, licensee understands it is to be posted in a prominent area for the next 30 days.
