Licensing Program Analyst (LPA) Sabrina Martinez arrived at the facility on 08/02/2018 at 08:36 am to conduct an annual random inspection at the facility. LPA met with Kristina Mitchell, Education Coordinator, who guided analyst on a tour of the facility.

LPA observed care being provided for infants in 5 classrooms which are located in the first floor of the facility. LPA observed 35 children being supervised by 12 staff members during this inspection. LPA verified that all adults present in the facility have obtained criminal record clearances and are associated to the facility. LPA also reviewed the sign-in and sign out sheets to verify the census.

All areas identified on the facility sketch were inspected. The following staff were present during this inspection:

RM 1: 4 infants with a ratio of 2 teachers
RM 2: 6 infants with a ratio of 2 teacher
RM 3: 7 infants with a ratio of 2 teachers
RM 4: 10 infants with a ratio of 3 teachers
RM 5: 8 infants with a ratio of 3 teachers

There were no baby walkers on the premises. Sanitary solution (bleach solution) is out of reach to infants in care. Changing tables have at least a 1-inch-thick padding covered with washable vinyl or plastic. Sides are raised to a minimum of 3 inches. Changing table is within an arm's reach of a sink. There is a minimum of 1 potty chair or toilet for every 5 infants currently toilet training. Toys are found to be safe with no sharp edges, splinter or points. There are no small parts that can be pulled off and swallowed. Cots are available for all infants who are able to climb out of a crib.

Cribs or other appropriate napping equipment is available for each crib age infant. Bedding is separately identified and stored for each infant. Placement of cribs, cots or mats allow for entry to/exit from the napping space.

The facility provides AM and PM snacks. Parents also bring the children’s snacks and lunch from home. The facility serves milk and water. The chemicals are kept separate from food. The children eat in the classrooms. Trash bins are covered or the trash is removed immediately.

Outdoor equipment was inspected for health, safety, cushioning material, good material, good repair and age appropriateness. There are several play areas within the playground. There are areas for shade and rest. Drinking water is available. Play area was inspected for hazards and inaccessibility to bodies of water. There are no bodies of water. There is a playground schedule for each program.

LPA observed the required postings. Classroom has one or more functioning carbon monoxide detector and smoke detector that meet statutory requirements. Licensee was observed to be operating within the conditions, limitations, and capacity specified on the license.

LPA observed staff records to contain appropriate documentation of education credits. At least one person was observed to be trained in CPR and Pediatric 1st Aid. Staff 1 has 1st Aid and CPR card which expires on 01/2020.



This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. New Immunization Requirement: Law enacted by SB 277, beginning January 1, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into child care or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.

New Appeal Process: A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment. All appeals must be sent to:

Assembly Bill 1207: California Child Care Workers; Mandated Training Requirement. Beginning January 1, 2018, all licensed providers, applicants, directors and employees must complete Mandated Reported Training prior to March 30, 2018 and renew training every two years at: www.mandatedreporterca.com.

Nutrition Requirement: Beginning January 1, 2016, AB 290 will require for each new license issued, at least one director or teacher at each child care center or family child care home to have at least one hour of training in the importance of childhood nutrition. This applies to anyone submitting a new application, relocating their facility, selling their facility or transferring their license. Please note this training cannot be completed online or by home study programs. The training must be taken from an Emergency Medical Services Authority (EMSA) approved training program OR an accredited college or university.

An exit interview was conducted with Silvia Hernandez, Assistant Center Director. A copy of this report along with a Notice of Site Visit were issued. The facility was made aware that all other Licensing reports must be made available to the public for 3 years.

Requirements for disaster drills and mandatory documentation for both children and staff files were discussed. Role and responsibilities of being a mandated reporter were discussed. The licensee was advised how to access forms and Regulations for Child Care Center online at www.ccld.ca.gov. Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised on how to access quarterly reports, forms, and regulations for Child Care Center online at www.ccld.ca.gov. The licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must have the facility’s phone number at all times; if the phone number is changed, licensing must be notified. Regulation prohibits the smoking of tobacco on the premises.


The Center Director was informed of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division. CCAP participates in many community activities and special projects in order to disseminate information on the State’s licensing role, provide information to the public and parents on child care licensing, and provide many other helpful resources to the licensees and the public. The following were also discussed with licensee:



Assembly Bill 633: Upon receipt of a citation for a Type A Deficiency, the director shall post the licensing report for 30 days in addition to the Notice of Site Visit & provide copies of the licensing report (LIC 809D/9099D) to parents/guardians of children in care at the facility by the close of business the following day or the next day child returns to the facility. The same report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months & licensee will obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from parent/guardian & place it in each child's file.

Senate Bill 792: This bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles.
