The Authorized Representative also stated she has daily communication from the staff about C1’s daily activities, has never had an issues with the facility, and has never observed bruises or marks on C1.”On 6/24/2020, LPAs Castellanos and Hill received records from the Director revealing that infant staff completed trainings on 5/6/2020 pertaining to children’s challenging behaviors, child discipline, and children’s personal rights.
Allegation 4 - Evidence provided to LPA Castellanos include photos that did not include a date or a diagnosis by a physician. Interviews with staff and parents revealed that the facility was not recently exposed to hand, foot, and mouth disease. The Director provided LPA Castellanos with a notification letter that would be given to the parents in the event of exposure.
Based on records reviewed, documents obtained, observations, and interviews conducted by the Department on the allegations were found to be Unsubstantiated. Unsubstantiated means that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violations occurred.
An exit interview was conducted. A copy of this report (LIC9099, LIC 9099C) and a Notice of Site visit was emailed to Director Natasha Madison. It has been explained that a reply to the email shall be considered a substitute for the hard-copy signature.
