On January 19, 2024, Licensing Program Analysts (LPAs) Carolyn Tuba and Jessica Hopkins-Hernandez conducted an unannounced required inspection. A COVID-19 risk assessment was conducted prior to entering the facility. LPAs entered facility through the Sunset elementary school’s front office and walked through a classroom and down the hallway towards classroom #2 (Head Start preschool program). LPAs met with On-Site Supervisor/Teacher, Mahvel Gala-Zuvala and an assistant who was subbing. LPAs took a census of 13 children and 2 staff in care. The program operates as Head Start preschool program. At approximately 10:30 am Site Supervisor, Ryan Martinez who oversees the preschool program and works off-site arrived and met with LPAs to facilitate the visit.

Hours of operation: The days and hours of operation for the Preschool Head Start: Monday-Friday, 8:00 A.M.- 11:30 A.M.

Location: In order to gain entry, the LPAs enter the main office of the elementary school. The classroom is located on the east side of Sunset Elementary’s main office. The classroom is in the school building and the playground is behind the preschool classroom and surrounded by a chain link fence, and brink wall, which is separated from the rest of the elementary school. The playground is shared with the school’s Occupational Therapy students and students are on a separate play schedule. The classroom is one large room with one restroom for the children, which includes 1 toilet, 1 urinal and 1 sink, and one staff restroom, a kitchenette, a portable refrigerator and one storage/office for the staff. Parents enter through a sliding glass door located off the staff and visitor parking lot and walk down the hallway. Parents may either enter through the main office or call ahead of time, so they are able to gain access to the classroom.

All posting requirements were observed on the Parent Board located outside of classroom.


LPAs viewed a copy of the monthly breakfast and lunch menus, which is posted on the teacher cabinet and outside on the parent board. Parents also get a breakfast and lunch emailed to them and is available on the school district website. All food is provided by the School District. There are currently no children on medication, but if medications are required then they shall be stored in a locked file cabinet inside of the locked storage room, which is labeled "Medication". LPAs observed First Aid Kit in the cabinet inside the classroom, which is labeled with a sign and LPAs inventoried and found all required items. The cleaning supplies are kept in the locked Storage room.

Staff and children's files were reviewed and found to be complete. LPAs, reviewed parents sign-in/out sheets and found to be complete. LPAs advised Site-Supervisor that parents need to use full signatures when signing in/out.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

There is a carbon monoxide and smoke detectors that are connected to the elementary school and are tested by the school district through-out the year. The last annual maintenance on the Fire Extinguisher was on September 7, 2023.

According to the on-Site Supervisor/Teacher children currently bring water bottles from home and refills are provided with a Brita pitcher that is filled using bottled water.

Site Supervisor was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Site Supervisor was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

There were no deficiencies cited during today’s inspection.

A notice of site visit was given and must remain posted for 30 days.
