Licensing Program Analyst (LPA) Judy Mora and Licensing Program Analyst (LPA) Crystal Green met with Gabriel Ross (owner) for Play Preschool who guided licensing staff on a tour of the facility. The purpose of this visit is to assess the proposed infant program. The proposed infant program will operate from 7:30AM- 6:00PM, Monday thru Friday. Applicant is seeking to provide care for 20 infants children ages 12 weeks to 24 months. The entire facility was viewed and toured. There is 1 infant classroom in total, and a napping room. The applicant is also applying for a preschool license, #198019498. The facility has 3 levels, the infant program will take place on the lower level, 2 levels having egresses to ground level. . The infant program is physically separate from the other components at this facility. The infant room is separated from the preschool room with a kiddie gate.

Per owner there are no weapons, firearms or bodies of water on the premises. Licensing staff did not observe any bodies of water during this visit. Disinfectants, cleaning solutions and other items that are dangerous to children are inaccessible to children.

There is a fully equipped first aid kit available on site. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. Sufficient individual storage space was observed for children. All materials and surfaces that will be accessible to children are toxic free. At this time, the Director's office will be used as an isolation area. Parents will be contacted immediately when children are determined to be ill. Age appropriate toys, material and equipment was observed.

The facility has Koala horizontal changing stations located in the Staff restrooms and are within an arms reach of a sink. A padded surface (1 inch thick) will be placed on the changing tables while in use. The napping room is separate from the indoor activity space classroom. Sufficient napping equipment was observed in the napping room, there are playpens and portable cribs available. High chairs were observed in the infant classroom.

Parents of infants enrolled will provide all food items to include milk, baby food and formula. All items will be labeled. A refrigerator is available to store all items. There is drinking water available in all indoor classrooms, water jugs and disposable cups.

Storage containers for solid waste, including moveable bins, are available and have tight fitting covers on and are in good repair. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements.

All toilets and hand washing facilities, are in safe and sanitary operating conditions. There is 2 toilets and 2 sinks inside the infant classroom. Staff restrooms are available on each floor.

Outdoor play equipment was observed to be in good condition, free of sharp, loose or pointed parts. Director was advised that the infant play ground needs to be resurfaced prior to licensing. The current flooring is not age appropriate and is just cement. Pictures will be sent to LPA. Director was advised that shade needs to be placed in the infant playground. A water jug and disposable cups will be taken outdoors.

Parents will be signing their children in right outside the Director's office. Parents will be signing in on an IPad which requires a signature, program used Brightwheel.

Medications shall be stored in a secured, inaccessible area. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Measurements were taken of all indoor activity and outdoor activity space. The following measurements were taken today:

Total indoor space: 744.45 sq. ft. /35 sq. ft. = 21

Total indoor activity space allows for 21 children.

Outdoor Activity Space Total: 1714.36 sq. ft. / 75 sq. ft = 22

Total Outdoor Activity Space allows for 22 children.

Fire clearance for this Preschool program was received by the Los Angeles Fire Department on 11/02/17, for the total capacity of 20 Children. The program will be approved and licensed for 20 infants upon corrections and file review.

The following corrections must be submitted to the Department by 11/13/17:

  • Infant playground needs to be resurfaced.
  • Infant playground needs to have a shaded area.
