7. Copy of Guardian Roster that includes Ms. Gonzales
8. Mandated Reporter Training
9. Lead Testing Training
10. Emergency Disaster Plan (LIC 610)
11. Personnel Report (LIC 500)
12. Designation of Facility Responsibility (LIC 308)-for current licensee.
A complete packet is to be provided to assigned LPA , G. Lucero.
During today's inspection a deficiency was cited under 101161(a) Limitations on Capacity A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation
Exit interview conducted and report was reviewed with the director, Monique Gonzales. Appeal Rights were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
