During the course of investigation, LPA interviewed 9 staff members, and 6 parents and 5 children interviewed.

During the staff interviews, Staff (S1), Staff 3 (S3), Staff 4 (S4), Staff 5 (S5) and Staff 7 (S7) disclosed that they have been over ratio. S3, S4 and S5 disclosed that children are often shuffled into different rooms because they are trying to maintain ratios and subsequently comingle the different age groups. S3 stated S3 has had up to 15 children and had to send some students to the preschool classrooms. S4 said S4’s class is often comingled with room #1 which has an age range of 1-2 ½ year old and that other classrooms are over in the afternoon. S4 stated comingling has been happening since March. S8 stated, sometimes in morning from 6am- 7am, the children are comingled. S5 stated they may have been over ratio only for a split second in when a teacher is coming in. S7 stated about being over ratio, 99% they are okay, but during the transition when another staff is set to come but it doesn’t happen often. S1 admitted that it (being over ratio) happens but not often, although the staff schedules are staggered to meet ratio needs but acknowledged that staggering aides is what is needed to meet any staff-child ratio needs. LPA obtained a sign in/out timesheets for students and staff. LPA observed comingling between infants, preschool and school-age. There was 1 infant, 2 preschoolers and 1 school-age child with one staff. Facility has a comingle waiver from 6am-7am but only for preschool and school-age it does not include infants.

LPA asked S1 if all the staff have met all the health requirements to work with children and S1 stated the staff do except Staff 10 (S10) does not have that in S10’s file right now. LPA reviewed staff 9 files, S10 did not have proof of TB test and health screening ready for review. S10 was hired in 04/10/2023.

On 6/21/23, LPA asked staff about the sink in room #3 not working, S1 stated it had not been working for a little over a week but that a plumber already came to take a look at it and the faucet was getting replaced. S4 disclosed that it has not been working since April 2023. S5 stated, It hasn’t been working for a month and there has been plumbers coming to fix it, but the leak can’t be fixed, perhaps it’s just too old and it needs to be replaced.” Child #1 (C1) the sink in the classroom had not been working for a little while. On 6/21/23 LPA observed that sink was not working and seemed to be leaking that same day a plumber came to the facility to take a look at the sink but would have to return at a later time to attempt to repair it.

6 of 8 parents were interviewed, parents had no issues or concerns with the daycare.

During the course of investigation, LPA interviewed 9 staff members, and 6 parents and 5 children interviewed.
During the staff interviews, Staff 1 (S1) stated, the snack they serve it is enough because we always have leftovers and if they ask for more we give them more. No, kids are never left hungry. Staff 5 (S5) said, They can always ask for more but we try to keep it a snack and not a meal. Parents know that they can provide additional snacks. There’s always enough. Staff (S6) disclosed, If the children ask for more they get more and said the kids are never left hungry. We make sure they eat, and children also bring their own extra snacks or we can ask for more from the kitchen.

When touring the facility LPA did not observe any toys in disrepair and observed age-appropriate meal sizes during snack and lunch.

6 of 8 parents were interviewed, parents had no issues or concerns with the daycare.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED. Exit interview was conducted, and report was reviewed and discussed. Notice of Site Visit was posted during the visit. The facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100 per day. The facility was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights.
