25 | Licensing Program Analyst (LPA) Malek conducted a case management inspection at this facility for the purpose of increasing capacity of infant program. The infant program is currently licensed for 16 infants and toddlers in two rooms. (Room # 1 and room # 4). Applicant has requested to increase their capacity from 16 to 18 children after some rearrangement in room # 4. LPA met with director, Fashana Afaz at the time of the inspection. LPA toured the site with Ms. Afaz. LPA observed one staff supervising two napping children in the crib area, one staff supervising 3 awake children in the activity area in room 4 and 7 toddlers were supervised with staff in room # 1.
This facility has sent a plan of operation for Incidental Medical Services- IMS to our office. A review of staff records on this date indicates that all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions. The director has done some rearrangement for this room by moving the napping crib area back in order to have room for two additional children. The total indoor activity space for two rooms are approximately 630 square feet that accommodates 18 infants and toddlers in two rooms. (Room # 1 and room # 4).There are a total of 9 toilets in the facility that is enough for 135 children. There are 3 sinks in infant /toddler rooms that is enough for 45 children. The outdoor playground is approximately 1844 square feet which is enough for 24 infants and toddlers. Playground is completely enclosed. Outdoor activity area is supplied with age and size appropriate equipment. There are adequate cushioning materials under the climbing structure. The Notice of Site Visit was posted. Facility representative was informed that the Notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.00. The licensee was provided a copy of their appeal right (LIC 9058 1/16) and their signature on this form acknowledges receipt of these rights. Exit interview was conducted. Copies of child care provider's guide to safe sleep pamphlet and Never Ever Shake a Baby pamphlet with the website www.dontshake.org were given to the facility representative. An updated license for 18 children will be issued for two rooms today. (Room # 1 and room # 4).
