1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Pg 1 of 3 An unannounced Required- 1 year inspection was conducted today by Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek who met with assistant director, Yvonne Smith. The director Fashana Afaz arrived during LPA's inspection. This facility has requested to increase their capacity from 18 infants and toddlers to 63 infants and toddlers by adding 3 additional rooms from suit # 34. The facility is currently licensed for 18 infants and toddlers in room # 1 and 4 (Room # 1 for toddlers and room # 4 for infants in suite 35). Today census was taken. There were a total of 16 infants and toddlers with 6 staff in 2 different rooms. There were three staff with 7 infants in infant room # 4. No infants was napping in the nap area. There were three staff supervising 9 toddlers in the toddler room # 1. The facility has added 3 additional rooms to this existing infant/toddler program to increase the capacity to 63 infants and toddlers. Suite #34 and 35 have been combined. The main entrance is from suite # 35 for infant/toddler and preschool programs. The fire clearance approval has been received in our office on 1/8/2019. According to the measurement of 10/23/2017 the existing rooms # 1 and 4 are approximately 630 square feet which accommodates 18 infants and toddlers. Today's measurement for 3 additional rooms are as follows: Room # 8 (infant room) 420 square feet after deducting the crib area Room # 9 (Toddler room) approximately 570 square feet Room # 10 (Toddler room) approximately 319 square feet Total capacity for these additional rooms are approximately 1309 divided by 35 =37 children The facility is licensed for 18 children, therefore the total measurement for the indoor activity for rooms 1, 4, 8, 9, and 10 are 1939 square feet which accommodates 55 infants and toddlers. (Rooms 4, and 8 for infants and rooms 1, 9, and 10 for toddlers). There are a total 7 sinks and 3 additional toilets in the new added space which are adequate for the requested capacity. According to the measurement of 10/23/2017 the playground measurement for infants and toddlers is approximately 1844 square feet which accommodates 24 infants and toddlers.

The facility has requested a waiver to use the playground for a total of 24 infants and toddlers at any given time. The facility has requested to increase their capacity to 63 infants and toddlers. Based on the total measurement, the facility can accommodate 55 infants and toddlers in rooms 1, 4, 8, 9, and 10.
An updated license will be issued after final review and approval of the waiver request for the playground.

A review of criminal record clearances indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

Today the facility was reviewed to ensure compliance with license conditions and limitations, staffing and ratios, inaccessibility to poisons, medication, and hazardous items that can pose a danger to children. Equipment and furniture was inspected to ensure it's in good condition, free of sharp, loose or pointed parts. The bins had tight fitting covers. The napping area is separated from the activity are and has sufficient infant napping equipment. There is no walkers on the premises. The changing table is within the arm's reach of a sink. Bottles and dishes are labeled with the current date and children's names. The infants and toddlers were being supervised in the awake area and the napping area. There are no weapons, firearms or bodies of water in the facility. The playground was inspected for safety, good condition of equipment, including appropriate cushioning material around and under high climbing equipment. Staff's files were reviewed for education verification, CPR/First Aid, infant/toddler units, and new immunization requirements for (MMR, Pertussis, and Flu vaccines).

A sample of children's files were reviewed for completeness of medical assessment, and verification of sign in/out including time the child was signed in/out by authorized representative as well as verification of representatives full legal signature. The facility has an individual feeding plan for each infant and each infant has an infant Needs and Services Plan.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual-Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The facility has sent the update plan of operation for providing IMS to our office. The Notice of Site Visit was posted. Facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The licensee was provided a copy of their appeal right (LIC 9058 1/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Licensing office within 15 business days. First level appeal is to Regional Manager, address is above on the report. The facility representative was informed that they can refer to our Department website at www.ccld.ca.gov for obtaining the quarterly updates.

Also provided was information about the E-Learning Modules available at https://ccld.childcarevideos.org

A copy of child care provider's guide to safe sleep pamphlet and a copy of Never Ever Shake a Baby pamphlet with the website www.dontshake.org were given to the facility representative on the last inspection date.
An updated pamphlet regarding safe sleep regulations in childcare and a pamphlet for lead poisoning facts were given to the director today.

The Mandated Reporter Training Certificates for staff were available on staffs' files.

In the areas that were evaluated, the facility was in compliance of the California Code of Regulations, Title 22, Division 12. No deficiency was cited.
