On 7/11/2024, the Orange County Child Care Office received a complaint alleging: 1) Infant sustained unexplained bruising while in care. 2) Staff did not follow physicians order regarding infant’s dietary plan.
During the investigation, LPA toured the facility, obtained Parent Handbook, reviewed live video footage, Personnel Report LIC500, Child Care Facility Roster LIC9040, reviewed completed Infant Toddler Schedules, Daily Infant Reports, Accident Injury Reports, Menu Plans, Daily Health Checks, sign in/out sheets, took photos, and conducted interviews with the Reporting Party. LPA called 9 parents to interview and 4 parents were available to be interviewed. LPA did not interview children; children had limited vocabulary and did not qualify for interview.
On 7/19/2022, 8/07/2024, LPA conducted an inspection for the purpose of investigating a complaint received on 7/11/2024 in the Orange County Child Care Office. During inspections a review of staff records on those dates indicated that all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions.
On 7/19/2024 at 11:50AM, LPA Navar toured the facility with Teacher/Admin Leslie Oliva. During tour of infant classroom LPA observed infants in the carpet area listening to music with 2 staff and 1 staff at the table area. Toys appeared to be intact, functioning, and free of sharp edges.
LPA inspected food storage for infants, each infant had an individual container labeled with their name. Bottles and baby food were stored in the refrigerator and labeled individually with infant’s name. LPA reviewed Individual Menu Plans with child’s name written. Menu Plans were located in food prep areas. During infant file reviews LPA reviewed completed Infant and Toddler Schedules, Daily Health Checks, Daily Infant Schedules, and Accident Injury Reports.
On 7/19/2024 LPA conducted interviews with 4 staff (S1-S4). S4 stated that classroom video recordings are on a loop and get recorded over, so there is no access to past dates. All staff interviewed stated that they communicate with parents daily regarding infant’s feeding plan and that copies of the feeding plan is kept in the classroom so that they can review it when needed.
