On July 13, 2023, Licensing Program Analysts (LPAs) Elyse Jones and Blanca Ruiz arrived at the facility to conclude the investigation regarding the above allegations. LPA conducted a tour of the facility and census was taken.

On June 28, 2023, a complaint was received alleging staff do not ensure children are adequately fed while in care and staff changed agreed upon services without notifying the child's authorized representative. It was alleged changes to food service was made without notification to authorized representatives and the facility was no longer serving afternoon snacks for some children enrolled. Prior to conducting the investigation, LPA was informed the facility reversed the change and was serving afternoon snacks to all children enrolled.

During the investigation, interviews were conducted with pertinent parties and records were reviewed, which included a 2023-2024 Family Handbook, June 2023 Snack Calendar, and a Classroom Schedule.

During interviews with pertinent parties, there was a recent change of facility directors. Current Director stated prior to the change, a discussion of upcoming changes took place, which included the discontinue of serving afternoon snack to children enrolled in the facilities Extended Day Attendance option. In this option, children are picked up at 3:00pm. Director stated she was unaware authorized representatives of those children enrolled were not notified. Director stated afternoon snack was not served to those children from June 13, 2023, to June 30, 2023. Director stated after reviewing the change, she decided to continue to offer afternoon snacks to all children enrolled.

LPA reviewed the facilities 2023-2024 Family Handbook. The handbook states afternoon snacks are served at 2:45 pm, 15 minutes prior to some of the children being picked up at 3:00 pm.

Based on documents reviewed and information obtained during interviews conducted, the preponderance of evidence standard has met; therefore, the above allegations are SUBSTANTIATED.

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Exit interview conducted, appeal rights were discussed, and this report was reviewed with Angela Twyman, Director.
